My inspection revealed that Rocky Mountain Drilling has drilled a 26 inch hole to a total depth of 100 feet and ran 100 feet of 20 inch casing (70 pounds per foot / inside diameter is 19.25 inches). Universal cemented the casing with 175 sacks of Type 1 cement with 2% CaCl and .125 pound per sack celloflake. The cement had a density of 15.6 pounds per gallon, a yield of 1.2 cubic feet per sack, and they used 5.2 gallons of mix water per sack. Universal pumped 37 barrels of cement slurry, 31 barrels of displacement, and there was 5 barrels of cement returns. My inspection revealed no violations at this time. |

I arrived on site and spoke with the security guard and he advised that cement trucks were not on site because the township road has been torn up and the trucks couldnt travel on the road. Rex Energy has a Road Use Agreement to come to the site off State Route 43 and Gallo Road ( Gallo Road is the road that has the issues). I went to the pad to talk with the Company Man ( Aaron Wilhite) and found that he was not in his office. I observed Summit Casings Truck was on site but no crews were with the truck. I also observed that the 9 5/8 inch casing was still on the pipe racks. While departing the site, I spoke with the security guard again and he advised me that the casing crew left sometime around midnight. NOTE: I tried to call the rig at 12:03am before leaving my house, because Aaron Wilhite had advised me earlier that they would be ready to cement at approximately 1:00am. I didnt get an answer when I called the rig, so I assumed Aaron was on the rig floor or rigging up cementers. |

We walked the entire perimeter of the site and found that the filter socks and silt fence are intact and functioning properly. We walked around the drilling rig and containment areas and found no spills on the containment or holes in the containment. Our inspection revealed no violations at this time. Seven producing wells are located on the pad (permit numbers (2528, 2530, 2498, 2554, 2486, 2553, 2529). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of zero psi. Identification was posted at the well and lease entrance. The production tanks and production equipment were surrounded by a lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. I tested the standing water for chlorides using a low-range QuanTab which concluded in a reading of less than 33 ppm. The access road was stable and drivable. Erosional controls were in place and appeared functioning.
Equipment on the pad consisted of the following: Fifteen 400 bbl production tanks; eight labelled as containing condensate and seven as containing produced water Seven gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower |

During the inspection, Unit #2 of the gas recovery unit had a leaking dump valve whose gas pressure fell below correct operating pressure and needed to be reset. Contacted the well tender to come out and reset the unit. |
