0262273187 | 2017-10-30 | Production Wells | No Violations | I was on location to conduct a status check of this well. The well is equipped with a plunger lift assembly. The pad is equipped with eight producing wells. Jersey barriers were installed around the wellhead. The tank battery consisted of 16, 400 BBL production tanks, and was surrounded by a steel dike. Other production equipment on the well pad consisted of:
8 sand separators 8 gas processing units 1 low pressure vertical separator 1 compressor unit 1 heat treater Identification was posted at the access entrance, the well, and the tank battery. The lease road was in good repair. No violations were observed at the time of inspection. |
0396456190 | 2014-04-01 | Drill / Deepen / Reopen | No Violations | A general inspection of the Holmes pad revealed that two of the eight conductors have been set. The site in general is in good shape. The road is complete and ninety percent of the pad is complete. The center still needs a top coat of limestone and the cellars have been cemented on the outside. Precision Rat Hole is the drilling contractor and O-tex is the cement contractor. No violations found. |
1126447087 | 2014-04-01 | Drill / Deepen / Reopen | No Violations | A general inspection of the Holmes pad revealed that this will be the last two of the eight conductors to be set. The site in general is in good shape. The road is complete the pad is complete. Precision Rat Hole is the drilling contractor and Allied is the cement contractor. The conductors were installed with no difficulty. No violations found. |
1207324183 | 2014-04-21 | Casing String Witnessed | No Violations | I was on location to observe the cementing of the 9.625 intermediate casing. No violations identified. |
1253485944 | 2014-04-19 | Casing String Witnessed | No Violations | I was on location to observe the cementing of the 13.375 surface casing. Surface casing was ran to a depth of 834 . Schlumberger cemented with 770 sacks of cement. Cement was returned back to surface in the amount of 25 barrels ( bbl). No violations were observed. |
1399136259 | 2014-04-18 | Drill / Deepen / Reopen | No Violations | I was on location to observe the drilling progress og Patterson Rig # 329. The hole was spudded on 4/18/2014 . The rig was drilling a 17.5 hole at 196 kelly bushing ( KB). No violations were identified. |
2126348540 | 2015-10-23 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of twelve, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed. |
