0317925452 2014-07-20 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. TopHole Drilling drilled a 26 diameter hole to 120. TopHole installed 120 of 20 OD casing. O-Tex Pumping was the cementing contractor. The cement was mudscaled, and O-Tex pumped 53.4 bbls of cement slurry at 15.6 pounds per gallon. The cement was displaced with 39 bbls of water. 19 bbls of cement returned to surface. Displacement was completed at 6:45pm.
0830031375 2015-10-08 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1175796889 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was unlocked at the time of inspection. The #6H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and equipped with locks on the man door. The man door was open at the time of inspection, but Chesapeake personnel were on site. The equipment on the pad consists of the following:

9 -400 bbl production tanks, three labelled as containing produced water, six labelled as containing condensate. 1- Low-pressure vertical separator located inside of the dike of the tank battery. 7 Producing wells 7 Sand separators 7 Gas Processing Units 5 Line Heaters 1 Heat Treater 1 Vapor Recovery Unit 2 Bullet Tanks

1179673709 2014-09-28 Casing String Witnessed No Violations I was on location at 4:30am to witness the cementing of the 13.375 surface casing. Nomac Rig # 73 drilled a 17.5 hole to a depth of 865 and Wyoming Casing set the surface pipe at a depth of 854. O-Tex Pumping cemented the casing with 164 barrels of Class H cement. 41 barrels of cement circulated to the cellar and weighed 15.3lbs/gal. on the pressurized scales. See cement report for more details on this job. No violations.
1352373544 2014-09-29 Casing String Witnessed No Violations I was on location at 2:00pm to witness the cementing of the 9.625 intermediate casing string. Nomac Rig # 73 drilled a 12.25 hole to a depth of 2018 and Wyoming Casing landed the pipe at a depth of 2008. O-Tex Pumping cemented the casing with 153 barrels of Class H cement. 21 barrels of cement circulated to the cellar and weighed 15.3lbs/gal. on the pressurized scales. 1968908295 2014-10-02 Drill / Deepen / Reopen No Violations I was on location with DOGRM Geo-Tech Anthony Carson to inspect the well pad prior to conducting a pre-site inspection this morning and get him familiarized with what a working pad looks like after its constructed. We met with Chesapeake Company Man, Wesley Waycaster, to inform him that we would be inspecting the outside and inside perimeters of the location. We walked the location and inspected the under-drains, berms, and drainage controls around the location. No violations.
