| 0734087878 | 2015-10-26 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:
Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed. |

There was approximately three inches of standing water inside the tank battery, which I tested for chlorides using a low-range QuanTab which concluded in a reading of < 30 ppm. No violations were noted at the time of inspection. |

Nomac Drilling, Rig 70, drilled a 17.5 borehole to 520 KB. Wyoming Casing crew placed 495 of 13.375, 54.5 lbs. / foot buttress thread casing into the hole. Casing hardware included 5 bowspring centralizers, 13.375 X 1.5 float shoe and 13.375 X 1.48 float collar. O-Tex Pumping LLC ( O-Tex ) pumped 30 barrels of gelled spacer followed by 5 barrels of water spacer. O-Tex batched a cement slurry up to 15.6 lbs. / gallon and proceeded to pump 104 barrels of slurry downhole. The displacement plug was drop and displaced to 476 KB using 74 barrels of water. The plug was landed @ 180 psi and subsequently bumped to 880 psi. This pressure was held for 5 - minutes and was within tolerances. Upon releasing the pressure a total of .5 barrel flowed back and stopped. All floats held. Cement samples were weighed going into the hole ( 15.5 ppg ) and returns to surface ( 15.6 ). A total of 16 barrels of cement returns were observed back to surface. No violations were identified during this inspection. |
