The surface-intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 211 psi. I performed a walk-around inspection of the pad, and no violations were observed at this time.

The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 213 psi. I performed a walk-around inspection of the production pad and no violations were observed at the time of inspection.
0679662483 2015-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.

0846932205 2014-11-05 Drill / Deepen / Reopen No Violations Tophole Drilling drilled a 26 hole to a depth of 120 and ran 120 of 20 94 #/ft. welded conductor casing. Express Energy Services pumped 10 barrels of fresh water ahead followed by 230 sacks of class A cement with 2% CaCl and .05 ppg of floseal. Displaced with 39 barrels of fresh water. 11 barrels of cement returned to surface.

1421418230 2015-01-13 Drill / Deepen / Reopen No Violations On 1/13/2014, Nomac Rig 77 drilled a 17.5 hole to a depth of 944 and installed 931.53 13.625 54.5 lb./ft. intermediate casing. O-Tex Cementing was the cementing contractor. O-Tex pumped 30 barrels of Gel spacer, followed by 10 barrels of water, follow by 840 sacks (179.5 bbl.) of Premium H 15.8 lb/gal cement. The plug was displaced with 136.7 barrels of water. Plug landed, floats held and there was 44 barrels of 15.4 lb/gal of cement returned to surface.
1423056328 2015-01-14 Drill / Deepen / Reopen No Violations On 1/14/2014, Nomac Rig 77 drilled a 12.25 hole to a depth of 1892.83 and installed 1889 of 9.625 40 lb./ft. intermediate casing. O-Tex Cementing was the cementing contractor. O-Tex pumped 30 barrels of Gel spacer, followed by 10 barrels of water, follow by 670 sacks (144.39 bbl.) of Premium H 15.8 lb/gal cement. The plug was displaced with 136.3 barrels of water. Plug landed incresed pressure to 1400 psi., floats held and there was 24 barrels of 15.2 lb/gal of cement returned to surface.
1423057305 2015-01-20 Drill / Deepen / Reopen No Violations On 1/20/2015, I performed a status check on the above location. At this time Nomac Rig 77 is drilling a 8.750 hole at a depth of 8862. Drilling permits and conditions are posted. No noted violations at this time.
