There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complet

1378829046 2015-01-03 Drill / Deepen / Reopen No Violations On 12/27/2014, Nomac Rig 77 drilled a 12.5 hole to a depth of 2161.70 and installed 2157 13.625 54.5 lb./ft. intermediate casing. O-Tex Cementing was the cementing contractor. O-Tex pumped 30 barrels of Gel spacer, follow by 760 sacks (163.78 bbl.) of Premium H 15.8 lb/gal cement. The plug was displaced with 156.92 barrels of water. Plug landed and pressure was increased to 1414 psi. floats held and there was 28 barrels of cement returned to surface.
1404268784 2015-11-16 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.

1524668621 2018-04-23 Production Wells No Violations I conducted a status check of the pad. The pad is equipped with eight producing wells, and partially drilled wells with conductor casing (# 22624 & 22623). The surf/int pressure gauge displayed a reading 16 psi, and the int/prod displayed 288 psi. The tank battery consisted of six -400 bbl production tanks. Other equipment on the pad consisted of the following.




The access road was stable. No violations were observed at the time of inspection.

1791270487 2018-09-17 Production Wells No Violations No equipment changes have been made to the pad since my last inspection. The surface-intermediate annular gauge displayed a reading of 17 psi, and the intermediate/production annular gauge displayed 531 psi. I performed a walk-around inspection of the pad, and no violations were observed at this time.
2047301797 2018-11-08 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate annular gauge displayed a reading of 16 psi, and the intermediate/production annular gauge displayed 431 psi. I performed a walk-around inspection of the production pad and no violations were observed at the time of inspection.
