0092676105 2015-11-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.

0132468699 2015-05-09 Casing String Witnessed No Violations I was on location to witness the 13.375 surface casing cement job. Schlumberger cemented 945 of new, 54.5lb/ft. surface casing with 182 barrels of Class A cement. 54 barrels of cement circulated to surface during the job and the floats held. See casing ticket for details on this job. No violations.
0379754774 2014-03-19 Casing String Witnessed No Violations I was on location to witness the cementing of the conductor casing. Top Hole Drilling augured a 26 hole to a depth of 120 measured from ground level (GL). 20 conductor casing was butt welded and set on bottom with holes cut at 3 and 6 off bottom to allow circulation. A 20 steel plate with a 2 ball valve was welded to the top of the casing. Express Energy Services rigged up to the 2 valve, tested the lines, and pumped 10 barrels of water to break circulation. They then mixed and pumped 48 barrels of cement slurry and displaced the slurry to 110 with 39 barrels of water. 8 barrels of cement circulated to the cellar. No violations. 0552119167 2015-05-12 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #71. They are currently drilling the 8.75 vertical section at a depth of 6,145. The projected mud up point is 7,335. I reviewed the 9.625 intermediate casing cement job report from 5/10/15 and the job went as planned with 28 barrels of cement circulating to surface. The location is in good condition and all drilling equipment is on containment. No violations.
1446349921 2015-05-19 Casing String Witnessed No Violations I was on location to witness the cementing of the 5.5 production casing. Schlumberger cemented the casing with 284 barrels of lead cement and 522 barrels of tail cement. They finished the tail slurry and released the dart plug. They displaced the plug with 384 barrels of water and the plug did not land. The company man received orders and they pumped 12 more barrels of water and did not bump the plug. I had them hold pressure on the casing for five (5) minutes to verify integrity. They released the pressure and the float did not hold and nine barrels flowed back. They pumped the flow back through the floats and sat on the cement for 6.5 hours until it had set up. 50 of the 60 barrels of Mudpush Express circulated to the pits during the job. 1489881982 2015-05-08 Drill / Deepen / Reopen No Violations I was on location to check on the rigging up status on the 10H well and review the 5.5 production casing cement job conducted on the 4H well on 5/7/15. I reviewed the Schlumberger cement reports and the job went as planned. The rig has centered over the 20 conductor casing and Nomac is rigging up to spud late this evening. The location is in good condition and all drilling equipment is on containment. No violations.
2022542634 2015-05-18 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #71. They have drilled the 8.5 lateral to a total measured depth of 17,367 and are currently tripping drill pipe out of the hole at a depth of 2,100 to run 5.5 production casing. The casing is on the racks and it is new, 20lb/ft., grade P-110 casing. I inspected the Tartan Completion System frac sleeves that will be run in the horizontal leg of the well and all the equipment is new. The location is in good condition and all equipment is on containment. No violations.
