The pad is equipped with three producing wells (permit numbers 22634, 22635, & 22637). This well is equipped with plunger-lift configuration. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of zero psi. Proper identification was posted. The tanks and production equipment were contained inside a steel lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. The access road was in good condition. Erosional controls were in place and appeared functioning.
Equipment on the pad consisted of the following: Nine 400 bbl production tanks; six labelled as containing condensate and three as containing produced water Three gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower |

Sidewinder Drilling Rig # 63 drilled a 12.25 bore hole to 2009 KB and placed 2006 of casing in the borehole. Halliburton cemented the 9.625 casing with 355 sacks of BondCem ( 15.8ppg, 2.3 cu/ft. / sack ). Mix water temperature was 56 F and pH - 7. Cement slurry was circulated to surface with a total of 10 barrels of cement slurry. Displacement plug pressure was bumped to 1500psi for 3.75 minutes due freezing in the lines affecting the ability to release the pressure. Upon releasing the pressure all floats held. No violations were observed during my inspection. |
| 1366138771 | 2015-01-09 | Drill / Deepen / Reopen | No Violations | I received a call ( 0817 hrs. ) from Jim Noon ( Rex Energy Waste Specialist III ) informing me that while filling the Synthetic Oil Based Mud ( SOBM ) tank MT004 the driver overfilled the tank causing SOBM to run out of the rear inspection hatch. With the weather conditions ( 15 degrees F & SW wind @ 11 mph ) the spill lacked mobility to travel far. Some minor amount ( 1/2 gallon )was cleaned up outside of containment and disposed in the half round waste tank prior to my arrival. I would estimate that approximately 1 to 1.5 barrels of SOBM were spilled onto containment and on the tanks. I prescribed to Aaron Wilhite ( R E Gas Development company man ) to have the tanks involved ( MT001, MT004 and MT003 ), containment area and any other surfaces affected by the release steamed and cleaned up with a super suction vacuum truck to remove all the SOBM. Mobilization is being delayed by the road conditions. Will follow up with further inspections. |
| 1416316312 | 2014-11-17 | Casing String Witnessed | No Violations | I was on the Kiko # 2 - H location to witness the placement and cementing of the 20 conductor casing. H A D Inc. drilled a 22.25 borehole to 120 GL with a Air Rotary drilling rig. H A D Inc. placed 116 ( 6 joints ) of 20, 88 lbs. / foot, welded casing in the borehole. The conductor casing had circulation windows cut into the pipe 5 feet ( @ 111 ) from the bottom of the casing for placement of the cement slurry. Universal Well Services ( UWS ) connected to the casing and established circulation with 10 barrels of fresh water. UWS mixed a cement slurry and circulated the slurry back to surface. Mix water temperature was 58 and pH was 7. The cement slurry consisted of 100 sacks of Type 1 with 3% CaCl2( blended ). I observed 7 barrels of cement slurry returned back to surface. Cement fell back 10 and will be topped off once the cellar is constructed. No violations were identified during this inspection. |

Sidewinder Drilling Rig # 63 drilled a 17.5 bore hole to 1020 KB and placed 25 joints of 13.375, 54.5 lbs. / foot casing in the borehole. Total run in pipe tally was 1011 . Float equipment included 1- 13.375 float shoe, 1- 13.375 float collar, 8 - bow sring centralizers. Halliburton mixed and circulated 900 sacks of Halcem cement downhole and back to surface. A top plug was used to displace the cement. Total displacement was 149.4 barrels. After the plug landed Halliburton increased the pressure by 500 psi for the green cement test. The casing passed thr green cement test and pressure was released with .75 barrels flowing back. I observed 50 barrels of cement returned to surface. No violations were identified during this inspection. |
