1000996461 2015-10-26 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1093893985 2018-11-25 Production Wells No Violations The well was equipped with a plunger-lift assembly. The surface/intermediate and intermediate/production annular gauges displayed zero psi. I performed a walk-around inspection of the pad and erosional controls were in place. No violations were observed at the time of inspection.
1421415026 2015-01-10 Drill / Deepen / Reopen No Violations Nomac Rig 70 drilled a 17.5 hole to a depth of 530 and ran 520 of 13.375 54.5 lb/ft J-55 surface casing, with 5 centralizers. O-Tex was the cement contractor. O-Tex pumped 30 barrels of gel spacer, followed by 490 sacks (104.72 barrels of slurry) of 15.6 lb/gal of class A cement. The cement was displaced with 74 barrels of water. Plug landed increaced pressure to 770 psi. floats held. There was 5 barrel of 15.3 lb/gal of cement returned to surface.
1421416901 2015-01-12 Drill / Deepen / Reopen No Violations Nomac Rig 70 drilled a 12.25 hole to a depth of 2510 and ran 2500 of 9.625 40 lb/ft J-55 intermediate casing, with 17 centralizers. O-Tex was the cement contractor. O-Tex pumped 30 barrels of gel spacer, followed by 920 sacks of 15.8 lb/gal of Premium H cement. The cement was displaced with 183 barrels of water. Plug landed increaced pressure to 1700 psi. floats held. There was 33 barrel of 15.6 lb/gal of cement returned to surface.
1423057600 2015-01-20 Drill / Deepen / Reopen No Violations On 1/20/2015, I performed a status check on the above location. At this time Nomac Rig 70 is drilling a 8.50 hole at a depth of 14482.74. At present they are tripping out of the well bore to replace there bit. Drilling permits and conditions are posted. No noted violations at this time.
1586666150 2017-08-15 Production Wells No Violations I conducted a status check of the location with DOGRM Tom Hill. The well is in production and was equipped with a plunger-lift unit. Equipment on the pad consisted of the following:

- 400 bbl production tanks; six labelled as containing condensate, and three labelled as containing produced water



The access gate and man doors were locked. Identification was posted at the well, the tank battery, and access. The lease road was in good repair. There was approximately three inches of standing water inside the tank battery, which I tested for chlorides using a low-range QuanTab which concluded in a reading of < 30 ppm. No violations were noted at the time of inspection.

The surface-intermediate and intermediate-production annular pressure gauges both displayed readings of zero psi. No changes have been made to the pad equipment since my last inspection. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
