0256994182 2015-11-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.

0431571793 2015-03-10 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #71. They are currently rotating and sliding in the 8.5 lateral section and preparing to trip out of the hole due to downhole motor failure. This will be the fourth motor change-out on this hole. Current measured depth is 13,793. The location is in good condition and all drilling equipment is on containment. No violations.
0919829895 2015-03-02 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing string. Nomac Rig #71 drilled a 12.25 hole to a depth of 1838 and Wyoming Casing landed the intermediate casing at a depth of 1828. Schlumberger cemented the casing with 147 barrels of Class A cement and 32 barrels of cement circulated to surface during the job. No violations. See casing ticket for more details on this job.
1131869090 2015-01-09 Casing String Witnessed No Violations I was on location to witness the setting of the conductor casing. Top hole Drilling augured a 26 hole to a depth of 120 and sat 120 of 20 casing on bottom with holes cut 5 off bottom to allow circulation. Express Energy Services cemented the casing with 39 barrels of Class A cement slurry. 10 barrels of cement circulated to the cellar. See casing ticket for more details on this job. No violations.
1429032880 2015-03-02 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Wyoming Casing is currently rigging up to run the casing. I inspected the casing and it is new, 40lb/ft., grade J-55. I will return in several hours for the cement job. No violations.
1986088235 2015-03-06 Drill / Deepen / Reopen No Violations I was on location to inspect the drilling operations of Nomac Rig # 71. They have drilled the 8.75 vertical section to a total measured depth of 7,200 and are currently laying down the 8.75 bottom-hole-assembly (BHA). They are going to pick up the 8.5 BHA and drill to the mud-up-point (MUP) of 7,431, and begin drilling the curve and lateral. The location is in good condition and all drilling equipment is on containment. No violations.
