0410848211 2015-02-23 Drill / Deepen / Reopen No Violations I was on location to inspect the drilling status of Nomac #71. They mudded up at 7515 and are currently drilling the 8.5 lateral section of the well at a total measured depth of 10,059. Estimated total depth is 15,881. There is new, 20lb/ft., P-110 casing on the racks. A independent contractor is tallying the pipe and inspecting the threads on every joint. The location is in good condition and all drilling equipment is on containment.
1134909930 2015-02-27 Casing String Witnessed No Violations I was on location for 5.5 production casing cement job. Nomac # 71 drilled the 8.5 lateral to a total measured depth of 15,881 and Wyoming Casing landed the production string at 15,879. Schlumberger rigged up to the Blackhawk cement head, tested the lines, and conditioned the hole with 60 barrels of Mudpush Express spacer. They cemented the casing with 306 barrels of lead cement and 445 barrels of tail cement. 40 barrels of spacer circulated to surface during the job. See cement report for more details on this job. No violations.
1428076967 2015-02-17 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Nomac Rig #71 drilled a 12.25 hole to a total depth of 1850 and Wyoming Casing set 1840 of intermediate casing in the hole. Schlumberger rigged up, tested the lines, and cemented the casing with 146 barrels of Class A cement slurry. They released the plug and displaced the plug to the float collar with 133 barrels of freshwater. The plug landed at 740psi and the pressure was increased and held at 1320psi for 5 minutes. The pressure was released and 1.0 barrels flowed back to the truck. 20 barrels of cement circulated to surface. No violations. See cement ticket for more details on this job.
1687532915 2015-11-10 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of eight, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. The well was in production at the time of this inspection, and the status needs changed in RBDMS from DR to PR. Preliminary restoration passed.
