0487587602 2015-05-27 Drill / Deepen / Reopen No Violations Nomac Rig 70 drilled a 17.5 hole to a depth of 840 and ran 836 of 13.627 OD 54.5 lb/ft J-55 surface casing. O-Tex was the cement contractor on location. O-Tex pumped 30 barrels of Gel Spacer followed by 5 barrels of fresh water, followed by 760 sacks ( 162.43 barrels ) of Premium H cement with 2% CaCl,0.2% X-AirP, 1/4 lb/sk Cello Flake. Started displacement after 30 barrels pressure went to 1000 psi could not pump displacement. Meet with O-Tex personal and Wade Gray and Tim Cantrell Drilling Forman and Drilling Superintendent for Chesapeake and I also called Northeast Region Manager Rick Anderson and explained to him what the proposal was to complete the surface cementing. O-Tex was supplying 1 tubing that was gone to be installed between the 20 conductor and the 13.625 surface casing to act a siphon string, as cement is pumped between the conductor and surface casing. O-Tex pumped 63 barrels of 15.6 / 1.18 yld class H cement. There was 10 barrels of cement returned to surface. Pulled 1 tubing. Nomac Rig 70 will drill to the float collar. Warrior will be on location to run a bond long.
1248356324 2017-11-13 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped an open flow Christmas tree assembly. Jersey barriers were installed around the wellhead and the well was identified with a legibly-marked well identification sign. The surface--pressure vertical separator. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

8- 400 bbl production tanks, three labelled as containing fresh water, and five labelled as containing condensate, with a low pressure vertical separator located inside of the dike. six producing wells six sand separators six gas processing units one heat treater one combustor unit two compressor stations one oil stabilization unit

Jersey barriers are installed around the production equipment.  The access road and man doors were locked.  Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road was stable in in good repair.
1428415433 2015-03-26 Drill / Deepen / Reopen No Violations Express Energy drilled a 26 hole to a depth of 120 and ran 120 of 20 94 lb./ft. of welded conductor casing. Express Energy was the cementing contractor. They pumped 230 sacks (48.34 bbl..) of class A cement 15.6 lb./gal. The cement was displaced with 39.07 barrels of fresh water. There was 17 barrels of cement returned to surface.
1437347178 2017-06-27 Drill / Deepen / Reopen No Violations I was on site during hydraulic fracturing completions of the #2H (PN#22665), #8H (PN# 22668, and #9H ((PN# 22667) wells. Halliburton was on site to complete remaining stages after 11/43 stages were completed in January or 2017, but were discontinued after a hole in the casing was encountered which was repaired in May of 2017. Halliburton was the completions contractor. I conducted a status check of the site while on location. No issues were noted at the well or pad at the time of inspection.
1438693580 2015-05-28 Drill / Deepen / Reopen No Violations Nomac Rig 70 drilled the cement that was in the 13.625 surface casing that was left in the pipe due to cement flashing. Thy drilled to 795. Warrior Energy Services was on location to run a cement bond log. It was determined that there was a good cement bond from 792 to 500. I called Northeast Region Manager Rick Anderson and it was also explained to him the bond log report. It was agreed that thy could continue drilling there 12.25 well bore.
1438694809 2015-06-01 Drill / Deepen / Reopen No Violations I performed a status check on the above location. Nomac Rig 70 was drilling the 8.750 well bore at a depth of 8838. We discussed the problem that O-Tex Cementing did not bring enough cement on location to cement the 12.25 well bore with 9.625 intermediate casing. This was noticed before they started pumping. Inspector Josh Sherring was on location at that time.
1498224150 2017-05-02 Completion Testing No Violations I was on location to witness the setting of the cement retainers on the above location. Nine Energy Services was the wire line contractor on location to set the retainers. Blake Riley was the consultant for Chesapeake. Nine Energy installed there wellhead equipment. They tagged the plug at 4500 they correlated their depth from this point. The top shot is at 4381 and the bottom shot is at 4383. The cement retainer was set at 4300. This will be used to do their cement squeeze.
1523789767 2018-10-31 Production Wells No Violations The well was equipped with an open-flow assembly. The surface-intermediate annular gauge displayed a reading of six psi and the intermediate-production annular gauge displayed 480 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
