-08---way float valves were installed in the bottom of the tubing. O-TEX pumped 10 bbl. of Mud Wash followed by 10.2 bbl. of Class H yield 1.15 cement at 15.8 ppg. and displaced with 22.8 bbl. of fresh water. I verified that the mix water temperature was 62 degrees and the pH was 6.0. I witness the cement mud scaled at 15.8 ppg. The tubing was bled off to confirm that the float valves held. The tubing was then pulled out of the well wet with 250-500 psi applied to the annulus to displace tubular volume with water-based mud to assure the float valves stayed closed. The casing was pressured up to 1393 psi and the well was shut in. Approximately 0.5 bbl. of cement was estimated to be squeezed away. The casing was then shut-in overnight. | |

-08---based mud. I witnessed cementing operations which started at 9:45 AM. O-TEX was the cementing contractor. O-Tex pumped 10 bbls of mud wash flush, followed by 12.3 bbls ( 60 sacks) of Class H cement with 0.3% FL 17 and 0.2% C45 that was mixed and pumped. I verified the cement weight at 15.8 ppg, along with the temperature of the mix water which was 65 degrees and the pH which was 7. O-TEX pumped the 12.3 bbls of cement followed by 3.4 bbls of water and 15 bbls of 14.5 ppg water-based mud to displace. Cementing operations shut-down and 3 bbls flowed back indicating a possible unbalanced plug. Mercer Well Services pulled 15 joints of tubing wet and then reverse circulated at approximately 1 bbm. Water-based mud circulated back indicating that the job was unsuccessful. Mercer Well Services then ran 15 joints of tubing back into the hole and proceeded to flush the casing of cement. The well was still circulating when I left site at 3:10 PM. | |

Nomac Drilling Rig #77 drilled a 12.25 borehole from 838 KB to 2499 KB on misted air. Wyoming Casing Service ran 56 joints of 9.625, 40 lb/ft casing to 2477 set at 2505 KB. The casing string was centralized with 16 - 9.9625 X 12.25 bow spring centralizers. O - Tex Pumping Services ( O - Tex ) conditioned the wellbore with 35 barrels of gelled spacer. O-Tex mixed 900 sacks of Class H cement and displaced it with a top plug and 183 barrels of water. The plug was landed and the pressure increased to 1575 psi ( 525 psi over final lift pressure ) and held for 15 minutes without any pressure decline. After releasing the pressure One (1) barrel of displacement fluid flowed back and stopped. All float held. I observed approximately 32 barrels of cement slurry ( 14.9 ppg ) returned to the cellar. No violations were identified during this inspection. |

Jersey barriers were installed around the wellhead and the well was identified with a legibly-marked well identification sign. The surface--pressure vertical separator. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:
8- 400 bbl production tanks, three labelled as containing fresh water, and five labelled as containing condensate, with a low pressure vertical separator located inside of the dike. six producing wells six sand separators six gas processing units one heat treater one combustor unit two compressor stations one oil stabilization unit Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair. |
