-Tex was the cementing contractor. Circulation was established at 8:43 AM and there was good fluid circulation throughout the cementing operation. A 35 bbl gel spacer was pumped ahead of the cement to condition the hole. The cement was mudscaled, and O-Tex pumped 198 bbls of cement slurry at 15.8 ppg. The plug was released at 9:37 AM and was displaced with 187 bbls of water as measured by pump truck tanks. Approximately 44 bbls of cement slurry returned to surface. The plug bumped the float collar at 10:03 AM and pressure was increased to 500 psi over and held for five minutes. The float held when pressure was released.

0054432436 2017-06-27 Drill / Deepen / Reopen No Violations I was on site during hydraulic fracturing completions of the #2H (PN#22665), #8H (PN# 22668, and #9H ((PN# 22667) wells. Halliburton was on site to complete remaining stages after 37/70 stages were completed in January or 2017, but were discontinued after a hole in the casing was encountered which was repaired in May of 2017. Halliburton was the completions contractor. I conducted a status check of the site while on location. No issues were noted at the well or pad at the time of inspection.
0503698613 2017-11-13 Production Wells No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped an open flow Christmas tree assembly. Jersey barriers were installed around the wellhead and the well was identified with a legibly-marked well identification sign. The surface--pressure vertical separator. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

8- 400 bbl production tanks, three labelled as containing fresh water, and five labelled as containing condensate, with a low pressure vertical separator located inside of the dike. six producing wells six sand separators six gas processing units one heat treater one combustor unit two compressor stations one oil stabilization unit

Jersey barriers are installed around the production equipment.  The access road and man doors were locked.  Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road was stable in in good repair.

A 30 bbl gel spacer was pumped ahead of the cement to condition the hole. The cement was mudscaled, and O-Tex pumped 166.7 bbls of cement slurry at 15.7 ppg. The plug was released at 12:18 AM and was displaced with 127 bbls of water as measured by pump truck tanks. Approximately 30 bbls of cement slurry returned to surface. The plug bumped the float collar at 12:32 AM and pressure was increased to 500 psi over and held for five minutes. The float held when pressure was released and 0.5 bbls of water bled back.

1435761104 2015-05-20 Drill / Deepen / Reopen No Violations I performed a status check on the above location. Nomac Rig 70 is drilling a 8.750 hole at a depth of 7150. All permits and conditions are posted. No violations at this time.
1445569906 2018-10-31 Production Wells No Violations The well was equipped with an open-flow assembly. The surface-intermediate annular gauge displayed a reading of zero psi and the intermediate-production annular gauge displayed 170 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
1490021791 2017-03-17 Completion Testing No Violations I was on location to witness the resetting of a new isolation plug. The plug that was set on 3/15/2017 did not hold the 7000 psi test for 10 minutes. The decision was made to tag the plug and set a new one. On 3/17/2017, 9 Energy Service was on location to set the plug. They went in the 5.5 casing with a 4.24 electronic cast iron millable plug. We tagged the plug at 7665 + 5 (7670). There was a 10 marker sub at 6535 that is where they correlated the measurement from before setting the plug at 7623 + 5 (7628). The plug was pressure tested in 1000 psi increments for 10 minutes starting at 0 to 1000 up to and including 9000 psi. After the step test was completed the pressure was bleed off to 0 psi and the well was blocked in to perform a negeative test. After 10 minutes the pressure was at 0. There was a chart used to record the pressure test and also the negetavite test.
1506357666 2018-06-19 Production Wells No Violations The well was equipped with an open-flow assembly. The surface/intermediate and the intermediate/production annular pressure gauges displayed readings of zero psi. I performed a walk-around inspection of the pad. No violations were observed at the time of inspection.
2145242686 2015-03-28 Drill / Deepen / Reopen No Violations Express Energy drilled a 26 hole to a depth of 120 and ran 120 of 20 94 lb./ft. of welded conductor casing. Express Energy was the cementing contractor. They pumped 230 sacks (48.34 bbl..) of class A cement 15.6 lb./gal. The cement was displaced with 39.07 barrels of fresh water. There was 16 barrels of cement returned to surface.
