-Tex was the cementing contractor. Circulation was established at 1:52 PM and there was good fluid circulation throughout the cementing operation. A 30 bbl gel spacer was pumped ahead of the cement to condition the hole. The cement was mudscaled, and O-Tex pumped 119.68 bbls of cement slurry at 15.6 ppg. The plug was released at 2:26 PM and was displaced with 87.87 bbls of water as measured by pump truck tanks. Approximately 22 bbls of cement slurry returned to surface. The plug bumped the float collar at 2:43 PM and pressure was increased to 500 psi over and held for five minutes. The float held when pressure was released and 0.5 bbls of water bled back.

1434545880 2015-05-25 Drill / Deepen / Reopen No Violations The H2S alarm sounded at 2:00 a.m on 5/25/2015 during drilling of the lateral on the Barnhart 8H well. I spoke Chad Franatovich, the company man, regarding what happened. The alarm showed 24 ppm (parts per million) at the half-round for collection of cuttings. The Nomac tool pusher ordered the annular to be shut in and the rig evacuated. The company man did a sweep of the shakers, pits, cellar, and half round. The hand held unit showed no H2S. TekSolv arrived on location and performed a sweep of the location. TekSolv found that the half round sensor in the half round was covered in water and cuttings. The H2S sensor was found to be damaged and was replaced. TekSolv then moved the half round sensor to the handrail on the intermediate pits.
1634978193 2015-05-20 Drill / Deepen / Reopen No Violations I performed a status check on the above location. Nomac Rig 77 is drilling a 17.5 hole at a depth of 1580. I instructed them to call . All permits and conditions are posted. No violations at this time.
1865997056 2017-03-03 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #8H well is in production and consists of a plunger lift unit. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, three labeled as containing produced water, and four labelled as containing condensate, and one low-pressure vertical separator. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

4-producing wells 8- 400 bbl production tanks, three labelled as containing produced water, and six labelled as containing condensate 4 sand separators 4 gas processing units 1 low pressure vertical separator 1 vapor recovery unit 1 horizontal separator 1 compressor station 1 line heater 2 bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.
