Seven producing wells are located on the pad (permit numbers (2528, 2530, 2498, 2554, 2486, 2553, 2529). This well is equipped with an open flow assembly. The surface-intermediate annulus was equipped with a pressure-relief valve and a pressure gauge which displayed a reading of zero psi. Identification was posted at the well and lease entrance. The production tanks and production equipment were surrounded by a lined steel dike. Less than an inch of standing water was present in areas inside the tank battery. I tested the standing water for chlorides using a low-range QuanTab which concluded in a reading of less than 33 ppm. The access road was stable and drivable. Erosional controls were in place and appeared functioning.
Equipment on the pad consisted of the following: Fifteen 400 bbl production tanks; eight labelled as containing condensate and seven as containing produced water Seven gas processing units with vertical separators Two combustor units Three vapor recovery units One oil stabilization unit and tower |

The access road was equipped with a gate and was locked. The #2H well is in production and is equipped with an open flow assembly. The production tanks are surrounded by a steel dike. A sound barrier wall was installed on the north side of the pad.
The equipment on the pad consists of the following: 7 producing wells 7 sand separators 7 gas processing units 1 vapor recovery unit 1 heat treater No violations were noted at the time of inspection. |
1450189429 | 2015-12-14 | Casing String Witnessed | No Violations | I was on the Grunder # 2H to witness the cementing of the 13.375 surface casing. Sidewinder Drilling Rig # 62 drilled a 17.5 diameter hole to 1014 KB. A water flow( approximately 300 - 400 barrels ) of water was encountered at ~ 710 KB. Kerzey Casing Service ran 22 joints of 13.375, 54.5 lb./ft., buttress thread and collar casing to 953 KB. Schlumberger conditioned the well bore with 50 barrels of gelled spacer followed by ten (10) barrels of fresh water. Schlumberger mixed and pumped 196 barrels of Class A cement slurry, displaced the slurry with 140 barrels of fresh water and a top plug. The plug landed at 445 psi and was bumped to 1025 psi for five (5) minutes without any pressure decline. One half (1/2) barrel flowed back and stopped. All floats held. No violations were identified |

All frac pumping equipment has been removed from location. Remaining equipment included sand storage silos, brine tanks and fresh water manifolds. Per Larry Boone ( co-man for Rex ) the drill out of the kill bridge plug should start on 1/28/2016 after which it is anticipated that 2.375 tubing will be ran in the 5.5 casing. Due to directional drilling problems, I requested Larry Boone to monitor the annular pressures of the 13.375 and 9.625 casing. No violations were identified during this inspection. |
