0588608554 2017-09-12 Production Wells No Violations I was on site in response to complaint #5671 regarding vibrating at the complainants house who lives near the pad. I received a call at 7:58 am on 09/12/17 from the complainant stating he was experiencing vibrations in his home. I arrived on site at approximately 9:00 am. I did not witness any equipment malfunctions or anomalies while on site. No other changes have been made to the pad since my last inspection. No violations were noted at the time of inspection.
0638531156 2015-09-14 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing string. Nomac Drilling Rig # 71 went in the 13.375 casing and tagged cement at 484 KB and drilled a 12.25 bore hole to 2409 KB. Wyoming Casing Services ran 56 joints of 9.625, 40 lb/ft, 8 round, long thread & couplings and 18 - bow spring centralizers. Total tally was 2,368 set at 2,388 KB. Schlumberger connected to the 9.625 casing and conditioned the bore hole with 25 barrels of gelled water followed by 10 barrels of fresh water. After the spacer, Schlumberger mixed a cement slurry consisting of 166 barrels of SurfaceSet ( 1.23 yield ) and displaced it back to the cellar with a top plug. Displacement volume was 175 barrels of water. Cement slurry weight going down hole was 15.4 ppg. The plug was landed at 1136 psi and increased to 1613 psi and held for five (5) minutes. The Green Cement Test was passed. After the pressure was bled off one (1) barrel of displacement fluid flowed back and stopped. All floats held. I witnessed approximately 20 barrels of cement slurry returned to the cellar. The returned cement slurry was scaled at 15.4 ppg. I inspected the oil based mud tanks, mud pumps, air package area and the sloids handling area and did not indentify any violations during this inspection.
0823412143 2016-09-06 Production Wells No Violations I conducted a follow-up inspection in response to compliant #5405. The access road was equipped with a gate and was locked. The #2H well is in production and the well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. No standing water was present inside of the dike.

1058247404 2015-07-22 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 110. Express installed 110 of 20 OD casing. During drilling, water was encountered at an approximate depth of 15. Express Energy Services was the cementing contractor. Water was pumped ahead to confirm circulation. The cement was mudscaled at 15.6 pounds per gallon (ppg). Express pumped 48.34 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39.07 bbls of water. Fourteen (14) bbls of cement returned to surface. Displacement was completed at 12:28 PM
1168578312 2018-11-24 Production Wells No Violations I was on site to follow-up on complaint #5671 regarding vibrating at the complainants house who lives near the pad. I observed no equipment anamolies or excessive noise/vibrations while on site. The surface/intermediate annular gauge displayed 25 psi and intermediate/production annular gauges displayed 180 psi. No violations were observed at the time of inspection.
1355065143 2017-02-01 Production Wells No Violations I conducted a follow-up inspection in response to complaint #5479. The #5H well is in production and the well is equipped with an open flow assembly. Sound barriers were installed around the two compressor units on site. No violations were found during my inspection.

The equipment on the pad consists of the following: 6-producing wells 15 - 400 bbl production tanks, five labelled as containing produced water, and ten labelled as containing condensate. 6 sand separators 6 gas processing units 1 vapor recovery unit 1 low pressure vertical separator 1 bullet tank that appeared not to be in production. 1 Line heater 2 heat treaters 2 compressors

1375755374 2017-08-02 Production Wells No Violations I was on site with Tom Hill in response to complaint #5671 in which the complainant, Mr. Kiefer, who lives near the pad reported incidents of house vibrations, and complaint #5479 regarding noise from the site. The noise from the compressor has been mitigated, and is audibly decreased after Chesapeake installed three walls of sound barriers have been around the compressor units. Complaint #5479 can be closed. We did not witness any equipment malfunctions or problems on the pad at the time of inspection. No changes have been made to the well or pad since my last inspection. There was approximately three inches of standing water inside of the tank battery which I tested for chlorides using a low-range Quantab which concluded in a reading under 30 ppm. No violations were noted at the time of inspection.
1396158214 2016-04-27 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. The well is properly identified with a legibly-marked well identification sign. The #2H well appears to be in production, but the well was completed December of 2015 and production records for the first quarter of 2016 are not yet available in the RBDMS database. Identification was posted at the tank battery which consisted of one low-pressure vertical separator, and 15- 400 bbl production tanks, ten labeled as containing condensate, and five labelled as containing produced water. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment.  The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire.  The access road and man doors were unlocked, but third-party personnel were on site during my inspection.  Drainage and erosion controls at the pad are in place and functioning.  The pad has a crushed limestone surface, and is in good repair.  The lease road was stable in in good repair.   No violations were observed during my inspection.

1768905350 2016-09-06 Production Wells No Violations I was on location to conduct an inspection of this well in response to complaint #5424. Identification signs were posted at the access road entrance, the well, and the storage tanks. The well is open flow with a wellhead Christmas tree assembly. The #2H well is in production. The tank battery consisted of 15- 400 bbl production tanks, ten labelled as containing condensate, and five labelled as containing produced water. The tank battery was surrounded by a dike and no standing water was present inside of the tank battery. Other production equipment on the well pad consisted of:

-frequency noise originating from the compressors was audible. Mark Deering of Chesapeake was contacted by DOGRM Regional Manager Tom Hill regarding the noise, and a sound study is being planned.

1820824009 2015-09-13 Casing String Witnessed No Violations I was on the Delmar 24-13-6 # 2H location to witness the placement and cement of the 13.375 surface casing and inspect the drilling activities. Nomac Drilling Rig # 71 tagged the conductor cement at 128 KB and drilled a 17.5 bore hole to 540 KB. Wyoming Casing Services ran 12 joints of 13.375, 54.5 lb/ft, Buttress Thread and Coupling and five (5) bow spring centralizers to 529 KB. Schlumberger connected to the casing and conditioned the bore hole with 25 barrels of gelled water followed by 10 barrels of fresh water. Schlumberger then mixed a cement slurry consisting of 104 barrels of Class A cement. The cement slurry was circulated back to surface with a top plug and 76 barrels of fresh water. The plug was landed at 570 psi and increased to 770 psi. After a static period of five (5) minutes, without any pressure decrease, the pressure was released and returned .5 barrel, then stopped. All floats held. I observed approximately 25 barrels of cement slurry was returned to the cellar. Cement slurry weight was verified going downhole at 15.5 ppg and the returns could not be scaled due to excessive foaming in the cellar ( due to the gel used to conditioned the bore hole ). A walk around inspection checked the solids handling area, Oil based mud tanks and mud pumps, the air package and the adjoining cellars. No violations were identified during during this inspection.
1934545128 2018-06-18 Production Wells No Violations I was on site in response to complaint #5671 regarding vibrations at the complainants house who lives near the pad. I received a call from the complainants residence at approximately 9:00 AM on 06/04/2018 stating vibrations were felt at the residence. I arrived on site at approximately 9:20 AM. No equipment malfunctions or anomalies were observed on site. I stood near the combustor for several minutes and did not hear or detect any anomalies at the time of inspection. No violations were observed on the pad.
