0499229172 | 2015-11-02 | Completion Testing | No Violations | I was on the Delmar 24-13=6 # 4H well to observe the hydraulic fracturing. The pad was constructed on an East to West axis. Entrance is on the Northeast corner of the pad. The stimulation monitoring trailer is also on the Northeast corner with Nine Inc. ( plug and perf contractor ) adjacent. The pumping equipment is located on the North and South sides of the wells with sand handling area to the West side of the wells. The acid and gel storage is on the Southwest of the pad. Performance Technology ( PTI ) was the fracing contractor. Two wells ( Delmar # 4H & 5H ) were being treated during my inspection. The Delmar # 4 well had experienced a screen out ( could not pump into formation further ) and PTI was flowing the 17th stage back in order to clear the perfs clusters. The screen out occurred while pumping a 15 lbs. crosslinked sand ladened ( 1 ppg ) fluid. After flowing back 75 barrels PTI started pumping back into formation at five (5) bpm @ 8400 psi. As the pressure increased ( 8900 psi ) the rate was brought down to four (4) bpm. Arrangement were made to bring in Coiled Tubing Truck to circulate out the obstructing sand. No violation identied during this inspection. |

The noise from the compressor has been mitigated, and is audibly decreased after Chesapeake installed three walls of sound barriers have been around the compressor units. Complaint #5479 can be closed. We did not witness any equipment malfunctions or problems on the pad at the time of inspection. No changes have been made to the well or pad since my last inspection. There was approximately three inches of standing water inside of the tank battery which I tested for chlorides using a low-range Quantab which concluded in a reading under 30 ppm. No violations were noted at the time of inspection. |

Identification was posted at the tank battery which consisted of one low-pressure vertical separator, and 15- 400 bbl production tanks, ten labeled as containing condensate, and five labelled as containing produced water. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

The access road and man doors were unlocked, but third-party personnel were on site during my inspection. Drainage and erosion controls at the pad are in place and functioning. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair. No violations were observed during my inspection. |

Water was pumped ahead to confirm circulation. The cement was mudscaled at 15.6 pounds per gallon (ppg). Express pumped 48.34 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39.07 bbls of water. Fifteen (15) bbls of cement returned to surface. Displacement was completed at 1:01 PM |
