1021311082 2017-06-29 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The #8H well is in production and consists of an open-flow assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of ten 400 bbl production tanks, four labeled as containing produced water, and six labelled as containing condensate, and one low-pressure. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

5-producing wells 10- 400 bbl production tanks, four labelled as containing produced water, and six labelled as containing condensate 5 sand separators 5 gas processing units 5 low pressure vertical separator 5 vapor recovery units 1 horizontal separator 1 compressor station 2 oil stabilization units. Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

1153416452 2018-11-19 Production Wells No Violations The well was equipped with an open flow assembly. The surface/intermediate annular gauge displayed a reading of zero psi and the intermediate/production displayed 45 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
1453523976 2016-01-21 Casing String Witnessed No Violations I was on the Edie 17-13-6 # 8H location to observe the cementing of the 13.375 surface casing. Nomac Drilling Rig # 71 drilled a 17.5 borehole from 140 KB to 580 KB. Wyoming Casing Service ran 547 of 13.375 casing to 572 KB. Schlumberger conditioned the wellbore with 25 barrels of gelled spacer followed by 10 barrels of water. Schlumberger circulated a cement slurry ( 112 barrels ) back to the cellar. Cement slurry weight was verified by scale at 15.6 ppg. Cement slurry returns to the cellar were 27 barrels. A walk around inspection of the drilling operations did not reveal any violations at the this time.
1484920142 2017-01-17 Completion Testing No Violations I received a call from Chesapeake Completions Supervisor Matt Wood on Saturday January 14th to notify me that hydraulic fracturing operations were going to commence on this pad within the next 24-36 hours. I was on location today to witness completion operations on this horizontal, Utica/Point Pleasant well. FTS International was conducting Stage 6 fracturing operations on this well and Stage 7 perforating operations on the #3H (PN# 22712). They pumped the proppant (sand) between 0.25lbs/gallon and 1.25lbs/gallon and at a rate between 80-83 barrels per minute. The hydraulic horsepower ranged between 17,300-17,600 horsepower. Average treatment pressure was 8549psi at 1.0lb/gallon and 8559psi at 1.25lbs/gallon. The maximum psi for this stage was 9138psi. The bottom hole pressure fluctuated between 10,600psi and 11,100psi during the stage. The annuli were being monitored during the job and readings showed pressures fluctuating between 200psi-207psi. When I left the job, FTS had put away 284,000lbs of sand. The job was going as designed.
1610379167 2015-10-21 Casing String Witnessed No Violations

1975065239 2016-01-22 Casing String Witnessed No Violations Nomac Drilling Rig # 71 drilled a 12.25 borehole 580 KB to 2110 KB. Wyoming Casing Services ran 52 joints of 9.625, 40 lb/ft casing, one (1) pup joint, one (1) float shoe, one (1) float collar, one (1) casing hanger , 17 bow spring centralizers and one (1) landing joint. Total tally ran was 2076 set at 2104 KB. Schlumberger cemented with a cement slury consisting of 761 sacks ( 166 barrels ) of Class A. Cement slurry returns were seen in the cellar and were estimated at 37 barrels. Cement weight was verified by scale at 15.3 ppg. After passing the green cement test ( 940 psi to 1500 psi, held for five (5) minutes ) one (1) barrel flowed back and stopped. All floats held. A walk around inspection of the oil based mud, back yard and location did not reveal any violations during this inspection.
