0803911161 2018-06-20 Production Wells No Violations I conducted a status check of the well and production pad. The pad is equipped with four wells (permit numbers 2718, 2719, 2720, 2721) and this well is equipped with an open flow assembly. Proper identification was posted at the well and lease entrance. The tank battery is surrounded by a steel dike. The enclosed tank battery and contained less than one inch of standing water. I tested the standing water for chlorides using a low-range QuanTab, which concluded in a reading of under 33 pppm. The access road was stable and drivable. I performed an inspection of the perimeter of the pad, and erosional controls appeared to be in place and functioning.

I did not locate a surface-intermediate annular pressure gauge. I contacted operator who stated they in the process of acquiring gauges from the supplier, and an annular gauge will be installed as soon as possible.

1473351970 2016-09-01 Drill / Deepen / Reopen No Violations I was on location to check the status of drilling operations on this Utica Shale well pad. I met with company man Clayton Lane and discussed operations. Appalachia Rig #62 (Sidewinder) has spudded the 8.75 production borehole and is currently drilling at a depth of 2529 with a rate of penetration (ROP) of 250/hour. The rig ran a survey and was on target. The location is in good condition and secondary containment is in place. The gauges on the Mathena (gas buster) were being serviced during my inspection. I found no violations at this time.
