-Tex was the cementing contractor. I witnessed good circulation during cementing. O-Tex pumped 18 bbls of water to establish circulation, followed by 49.16 bbls of Class A cement. I witnessed the cement mudscaled at 15.6 ppg. The slurry was displaced with 34 bbls of water. Approximately one bbl of cement circulated to surface. I verified through the O-Tex cement job report that a compressive strength of 500 psi is reached after nine hours. Chesapeake does not anticipate to start into the hole for at least twelve hours.

0396416002 2018-01-11 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The pad houses three producing well. This well is open flow with a wellhead Christmas tree assembly. The surface--lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

Nine Storage tanks, six labelled as containing condensate, and three labelled as containing produced water1 Producing well one low-pressure vertical separator three sand separators three gas processing Units one combuster unit one oil stabilization unit Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0399213243 2017-08-10 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 571 drilled a 17.5 inch diameter open hole to a depth of 481 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 135 feet. Wyoming Casing ran 475.2 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot, leaving a 5.2 foot stick up. Casing was centralized with 5 centralizers. Shoe track is 43 feet long. Schlumberger was onsite with 440 sacks of class A cement, surface set blend. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 67 barrels of fresh water. 25 barrels of gel pumped ahead of cement. 26 barrels of cement returned to surface, and floats held with .25 barrels back. Mix water has pH of 7 and 62 ppm chlorides. Pressure test was unsuccessful due to a leak on the casing swedge. Rig will pressure up to a minimum 300 psi and hold for 30 minutes before drilling out.
0881955434 2017-11-01 Drill / Deepen / Reopen No Violations I was on site to check the status of completion operations. I spoke to the consultant on site, who informed me that hydraulic fracturing was completed on the three drilled wells on the pad (#6H, 7H, & #9H) wells during the week of 10/23/2017, and Pro-Frac was the contractor. A service rig was in the process of drilling out plugs on the three wells, and flow-back operations were expected to take place within the next couple of weeks.
1924100763 2017-09-14 Drill / Deepen / Reopen No Violations I was on site to check the status after Patterson #571 moved off site. The rig and all drilling equipment has been removed from site. I have not received indication from the operator as to when completion operations will take place. No violations were noted at the time of inspection. I performed a walk-around inspection of the pad, and did not locate any issues of sedimentation and erosion. No violations were noted at the time of inspection.
