Cement circulated to surface and stayed at surface. They advised that they would like to leave the tubing in the well at this time. I advised since there was cement on the inside and the outside of the tubing that this was acceptable. Checked the well again before leaving the site and the cement was still at ground level. I advised that the well head / casing had to be cut off at least 30 below grade. I was advised that they were planning on cutting it off at least that deep and possibly deeper since they were going to construct a parking lot on top of the plugged well. There were no signs of any gas leakage at the time of our departure. Well has been properly plugged and abandoned at this time. No violations of Ch 1509 ORC or Ch 1501 OAC at this time. |

Since there was cement on both the inside and outside of the tubing. I agreed to the request. We checked the well before leaving the site and found the cement was still at ground level and there was no signs of gas leakage. I advised the plugging crew that the wellhead / casing had to be cut off at least 30 below grade. They stated that they were planning on cutting it off at least that deep and possibly deeper since there were plans to construct a parking lot on top of the plugged well. No violations of CH 1509 ORC or CH 1501 OAC at this time. |
1421846402 | 2015-01-20 | Final Restoration | No Violations | I was onsite to conduct a status check requested by Erica Freeman. The well is listed in the state database as plugged and abandoned. A permit to plug the well was issued 5/18/2011. Inspector Brett Farnsworth reports witnessing the cementing of well on 7/7/2011 from bottom to top with cement circulating to surface. He state the well was to be cut off well below 30 inches because a parking lot was going to be installed on location overtop. Onsite, I saw no evidence of any well and the location is now the parking lot of the Columbiana Oil and Gas Company of Ohio. No evidence of tank battery or lines or any other production equipment could be found. Final Restoration has passed. |
