| 0378346219 | 2014-08-12 | Storage Well | No Violations | I inspected Lake Shore Gas Storage well # 1978 today. The well is located in Section 13, Knox Township, Columbiana County, Ohio. The well is equipped with a master gate valve, a side gate valve plumbed into a 4 main line. There is a flow gate with a 3 line plumbed into a 100 BBL storage tank. All lids and hatches are closed and secured. Two (2) pickup lines on the storage tank and they are secured with valves and bullhead plugs. The identification sign is faded and needs to be corrected. The lease road is in stable condition. I have contacted the company about the issue. |

Correct identification was posted at the well head. The injection casing was equipped with a master gate valve that was topped with a flow cross. The top of the cross contained an observation port. The sides of the cross contained wing valves. The East wing was connected to a 3.5 run. The West wing was connected to a 4.5 run. No leaks were observed. All valves were closed. A domestic supply connection was present. A stable earthen dike that was free of fluid was present adjacent to the well head. The steel fluid tank located inside the dike remained in good repair. Valves were closed and plugged. No violations were observed during this inspection. |
