0306335156 2017-04-06 Production Wells No Violations The purpose of my inspection was to witness the posting of correct owner identification on well #2-0874. Lease operator Pep Horning updated the owner on the well head and tank battery identification signs. No further action is necessary.
0569839805 2017-03-29 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0874. Lease access to the well was stable. The well location was within a reclaimed strip mine and was stabilized with unharvested soybeans. No identification was present at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. The main gate valve was in the closed position. No audible or visual leaks were detected. The tank battery consisted of one (1) horizontal separator and one (1) 210 bbl production tank. An earthen dike that was stable but worn was present around the production equipment. The separator was in good repair. No audible or visual leaks were observed. A current 16 day production chart was in the meter house. Identification present on the production tank did not reflect the current owner of record. The tank body exhibited extensive exterior corrosion. No holes or leaks were observed. The load line valve was closed and plugged. The drain line valve was closed and plugged. The hatch lid was closed. I spoke with lease operator Pep Horning while leaving location. Mr. Horning stated that the well was shut in due to pipe line maintenance. I addressed the missing and incorrect identification at the well head and production tank. Mr. Horning stated that he would have the situation corrected within a week. I will schedule a follow-up inspection to document correction of the identification signs.
0633647169 2014-12-31 Production Wells No Violations I was on site to conduct a status check. The wellhead and tank battery have separate access entrances. The wellhead access road entry is on the north side of SR 30 approximately 0.36 miles west of SR 9. The tank battery access road entry is on the south side of Campbell Road (CR 840) approximately 0.03 miles east of Mardis Road (CR 826). The tank battery access GPS coordinates are 40.75348, -80.961308 +/-9ft. Identification was posted on the tank battery and wellhead. The tank battery consists of one 210 bbl production tank and one horizontal separator. The tank valves are plugged. There is a meter box with a current chart showing production. The dike is present and in fair condition. The wellhead is equipped with a plunger lift. The access road and site are stable. No violations were observed.
