0393584708 2017-04-24 Production Wells Violations Noted The purpose of my inspection was to conduct a status check on well #2-0954. ODNR DOGRM Northeast Regional Supervisor Thomas Hill was present at the time of inspection. A review of RBDMS indicates the owner is current with reporting production. Lease access was from a farm driveway into an agricultural field. The well location was stable. Correct and legible identification was present at the well head. The 8.625 casing head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. The stuffing box was leaking gas due to worn stuffing box rubbers as a result of a misaligned pumping unit. Condition of the well head indicated the condition had been ongoing. The tank battery was shared with well #2-0955 and consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. The separator was positioned outside of the earthen dike containing the tank and vault. A current 16 day production chart was in the meter house. The ground union on the dump valve appeared to have been recently replaced and was leaking oil onto the floor of the separator. Hammer unions on flow lines coming into the separator appeared to have been recently replaced and were leaking production fluid onto the ground. Identification posted on the production tank identified the Herold #2 and Herold #3, but only identified permit #2-0955. The production tank had been inverted. No holes or leaks were observed on the body of the tank. The load line valve was closed and plugged. The drain line valve was closed and plumbed to the brine vault. The concrete brine vault was in good repair above grade. No evidence of overfilling was observed. Current operation of the well is causing contamination and pollution. 1493985330 2017-04-14 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0954. Access to the well and tank battery was through a farm driveway. The road was stable and drivable. A review of RBDMS indicates the well owner is current with reporting production. The well location was in an agricultural field and was stable with well-established vegetation. No identification was present on the well head. The 8.625 casing head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. The pump jack was misaligned, being too far forward over the well head and caused excessive wear to the stuffing box rubbers. The wear caused the well to be leaking gas out of the stuffing box head. Some production fluid had leaked down over the well head and collected on the ground. The fluid was not fresh or live. A dikes area was constructed around the well head. The tank battery was shared with well #2-0955 and consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. The separator was positioned outside of the earthen dike containing the tank and vault. A current 16 day production chart was in the meter house. The ground union on the dump appeared to have been recently replaced and was leaking oil onto the floor of the separator. 1495737632 2017-05-23 Production Wells No Violations The purpose of my inspection was to follow up on repair work performed in response to compliance notice #393584708. Correct and legible identification was posted on the pump jack frame. The well head leak was repaired. The identification sign on the production tank was corrected to designate both wells (#2-0954 and #2-0955) that produced into the tank. Flow line hammer unions into the separator were not leaking. The ground union inside the separator was not leaking. The owner rebuilt the dike around the production tank and concrete vault. The load line valve and drain line valve were dry. No further action is necessary.
