ODNR DOGRM Northeast Regional Supervisor Thomas Hill was present at the time of inspection. A review of RBDMS indicates the owner is current with reporting production. Lease access was from a farm driveway into an agricultural field. The well location was stable. No identification was posted at the well head. The 8.625 casing head was below grade. The 4.5 production casing was equipped with an electric pump jack. Production valves were in the open position. A tubing pressure gauge indicated 0 psi. The stuffing box was leaking production fluid onto the well head and collecting and migrating on the ground. The condition of the area indicated the leak was not recent. A large collection of used drive belts and other refuse remained on the pumping unit. The drive belt guard was lying on the ground behind the unit. The tank battery was shared with well #2-0954 and consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. The separator was positioned outside of the earthen dike containing the tank and vault. A current 16 day production chart was in the meter house. The ground union on the dump valve appeared to have been recently replaced and was leaking oil onto the floor of the separator. Hammer unions on flow lines coming into the separator appeared to have been recently replaced and were leaking production fluid onto the ground. Identification posted on the production tank identified the Herold #2 and Herold #3, but only identified permit #2-0955. The production tank had been inverted. No holes or leaks were observed on the body of the tank. The load line valve was closed and plugged. The drain line valve was closed and plumbed to the brine vault. Contaminated soil excavated from the well head area was removed from location and the well head area backfilled with clean material. The well head leak was repaired. Discarded refuse was removed from the well location. The identification sign on the production tank was corrected to designate both wells (#2-0955 and #2-0954) that produced into the tank. Flow line hammer unions into the separator were not leaking. The ground union inside the separator was not leaking. The owner rebuilt the dike around the production tank and concrete vault. The load line valve and drain line valve were dry. No further action is necessary. |

1560162164 | 2017-05-08 | Production Wells | No Violations | The purpose of my inspection was to witness the excavation of production fluid contaminated soil from the well head area documented in Inspection Report #847147135. I met Northwood Energy Corp North Area Field Foreman Chris Schofield on location and observed Mr. Schofield direct the excavation of material until clean material was reached. The excavated material was stored on a pit liner. Mr. Schofield stated that he was going to regrade the access road and use the material to backfill the well head area. Once backfilled., Mr. Schofield planned to clean the well head, install new stuffing box rubbers and adjust the bridle from impacting the stuffing box. Following the well head excavation, an area within the diked area of the tank battery was excavated to determine the presence of production brine contamination. A pit approximately twenty-four inches deep and 12 inches wide was excavated adjacent to the production tank. The diked area was holding freshwater within the gravel base. No evidence of a recent production brine leak was observed. Mr. Schofield also had a contractor on location to replace the drain line valve on the production tank and tighten the hammer and ground union fittings on the separator. The Owner also updated the identification sign on the production tank to reflect both wells operated in the tank battery. I will plan to re-inspect the location to document the condition of the regraded well head and equipment repairs. |
