0348237689 2017-06-26 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1096. RBDMS indicated current production. The well head and tank battery were stable and in good repair. Correct and legible identification was posted at the well head. The 8.625 well head was above grade. The annulus was plumbed above grade and I could audibly detect gurgling from the pipe. The 4.5 production casing was equipped with a plunger lift assembly. The tank battery consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. The separator remained in good repair and contained a current 16 day production chart. Correct and legible identification was present on the tank body. The tank body remained in good repair. Valves were closed, locked and plugged. The vault was in poor repair. The side of the vault was broken out and contained water at freeboard level. I tested the water with a low range Quantab that indicated 7.6 on the test strip or 552 mg/L. An earthen dike that was stable and free of fluid was constructed around the equipment. I spoke with Atlas Field Superintendent Rusty Morrison concerning the condition of the vault. Mr. Morrison stated he would schedule to have the vault emptied and removed from the tank battery within 14 days. I will schedule a follow up inspection to confirm the vault had been removed.
0359313831 2015-10-06 Production Wells No Violations I was on site to conduct a status check. Identification was posted on the tank battery. The tank battery consists of one 210 bbl production tank, a concrete brine vault, and one horizontal separator. The tanks valves are sealed with a plug and locked. There is a meter box with a current seven day chart showing production. A dike is present. The wellhead is equipped with a plunger lift unit. The access road and site are stable.
0725755600 2017-06-26 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1096. Access was through a residential driveway. The well head and tank battery were stable and in good repair. Correct and legible identification was posted at the well head. The 8.625 well head was above grade. The annulus was open to atmosphere and audibly making gas. The 4.5 production casing was equipped with a plunger lift assembly. The tank battery consisted of one (1) horizontal separator, one (1) 210 bbl production tank one (1) concrete brine vault. The separator remained in good repair and contained a current 16 day production chart. Correct and legible identification was present on the tank body. The tank body remained in good repair. Valves were closed, locked and plugged. The vault was in poor repair above grade. The vault body contained a hole in the side and was leaking water. I field tested the water for chlorides and observed a result of 7.6 with a low range Quantab. No fluid was pooling inside the dike. An earthen dike that was stable and free of fluid was constructed around the equipment. I spoke with Atlas Field Superintendent concerning the condition of the brine vault. Mr. Morrison stated he would have the vault removed and the dike backfilled within 30 days.
0814330411 2017-07-27 Production Wells No Violations The purpose of my inspection was to follow up on work perfomed to remove the failed concrete brine vault. The vault area was back filled and the South wall of the earthen dike was rebuilt and stable. Vegetation was not yet established. No further action is necessary at this time.
0890013319 2017-07-19 Production Wells No Violations The purpose of my inspection was to witness the removal of the failed concrete brine vault from the tank battery on well #2-1096. Mahoning County Inspector John Fleming was present during the inspection. I arrived on location to find Bruderly Excavating finishing the removal of the vault. Harold Austin was on location to represent Atlas. I did not observe any leakage of production fluid into the subgrade of the battery. Bruderly planned to return to the location with fill material to backfill the excavated area. I will plan to re-inspect the location once the location is backfilled.
