0878668114 2017-06-05 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1339. A review of RBDMS indicated the owner was current with reporting production. Access to the well head was through the Ellyson lease to the South (#2-0368). No accessible road was present to the well head. The well was located adjacent to an agricultural field. The location was stable with well-established vegetation. No identification was present at the well head. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No audible or visual leaks were observed. There was a rubber patch clamp on the tubing. The tank battery was shared with well # 2-1343 and #2-1338. Production equipment consisted of one (1) horizontal separator and one (1) 100 bbl production tank. The horizontal separator remained in good repair. No audible or visual leaks were observed. A current 31 day production chart was in the meter house. No identification was present on the production tank. The load line valve was closed and plugged. The drain line valve was closed and did not contain a plug. The condition of the valve was not of recent use. The hatch lid was closed. I called and spoke with contract well tender Jeremy Kolar to request identification be posted at the well and tank battery within one week.
1281857605 2017-08-17 Production Wells Violations Noted The purpose of my inspection was to document the posting of correct identification at the well-head and tank battery. At the time of my inspection correct identification was not present at the well-head. Identification at the tank battery identified the operator and emergency phone numbers. Well #2-1339 is in violation of OAC 1501:9-9-05(A)(10) All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank(s) as to owner, lease name, well number, permit number, county and emergency telephone number. Petrox Inc. shall post correct identification at the well-head and update identification at the tank battery within seven (7) days of receipt of this compliance notice. Petrox, Inc. shall contact Inspector Kenneth Frye to confirm posting of identification.

1946998427 2015-08-06 Production Wells No Violations I was on site to conduct a status check. The tank battery is shared between the Gray #2 (#21338), Gray #3 (#21339), and the Gray #4 (#21343). Identification was posted on the tank battery. The tank battery consists of three 100 bbl production tanks and three horizontal separators. The tanks valves are sealed with a plug. There is a meter box with a current chart. A dike is present. The wellhead is equipped with a plunger lift. The access road and site are stable. No violations were observed.
