1202363674 2004-05-04 Production Wells No Violations The tank battery is shared with permit 1355 and consists of 1-120 bbl storage tank, 1-buried concrete salt water tank and a dike.
1457307479 2015-06-30 Production Wells No Violations I was on site to conduct a status check. The tank battery access is separate from the wellhead access and is located at 40.81379, -80.98147 +/- 10 ft. Identification was posted at the tank battery. The tank battery is shared between the Blasiman #1 (21354) and the Blasiman #2 (21355). The tank battery consists of a 210 bbl production tank, a concrete brine vault, and a horizontal separator. There is a meterbox with a current 31 day chart showing production. A dike is present and in good repair. The wellhead is equipped with an electric pumpjack and a domestic supply line. The access road and site are stable. No violations were observed.
1725524804 2017-07-03 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1354. The lease road, well and tank battery were stable. The 4.5 production casing was equipped with an electric powered pump jack. The pumping unit was operating at the time of inspection. Correct and legible identification was present. No leaks were observed. A domestic supply connection was present. The tank battery was shared with #2-1355 and consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) concrete brine vault. The separator was in good repair and contained a current 31 day production chart in the meter house. An earthen dike that was stable and free of fluid was constructed around the production tank and vault. Correct and legible identification was present on the tank body. Valves were closed and plugged. No leaks were observed. The vault remained in good repair above grade. Some staining consistent with legacy operations was present at the brine vault. The staining was not fresh or live. The vault was holding fluid with freeboard. No violations were observed during this inspection.
