0066150878 2016-07-25 Production Wells No Violations The purpose of my inspection was to conduct a follow-up visit with Northeast District Supervisor Ricahard Anderson concerning the hole in the storage tank and insuffcient earthen dike. At the time of inspection, both the tank and dike remained in poor repair. Supervisor Anderson concurred that the condition of the tank and dike were of concern to public health and safety and were in need of repair/replacement. I will plan to follow up again with Mr. Horning concerning the necessary repairs.
0177921413 2015-04-16 Production Wells No Violations I was onsite to conduct a follow-up inspection. On 12/31/2014 I conducted a status check where I observed that the dike was worn and needs to be raised in one area. I contacted Pep Horning, pumper for S&S Energy, on 1/6/2015 regarding the violation. Upon reinspection, the dike issue remains and has not been resolved. I will recontact Mr. Horning regarding the remaining issues.
0313442641 2016-11-15 Production Wells No Violations I received a voicemail from lease operator Pep Horning on 11/15/2016 indicating the production tank had been replaced with a 50 bbl poly brine tank. I re-inspected the tank battery location and found the steel production tank to have been replaced with a poly unit as stated by Mr. Horning. The tank was in good repair with no cracks or leaks. The load line valve was closed and plugged. No further action is necessary.
0408722363 2016-06-30 Production Wells No Violations The purpose of my inspection was to conduct a follow-up inspection regarding Complaint # 5331. I previously inspected this location (Inspection # 1470317524) and found the tree had not been removed. I received a voicemail message from Pep Horning indicating the tree had been removed. Upon my inspection I observed that the tree had been removed and the security fencing repaired to prevent unauthorized access. Neither the hole in the storage tank or the insufficient earthen dike concerns observed during that inspection have been addressed at this time. I will discuss the situation with management and proceed accordingly. I will also update the Complaint Log to close the complaint regarding the fallen tree.
0739186471 2016-10-24 Production Wells No Violations I called and spoke with lease operator Pep Horning concerning the status of the repair or replacement of the production tank. I communicated to Mr. Horning that the holes in the tank body side would not prevent the escape of production fluid creating a clear and present hazard to life, health and property. Mr. Horning acknowledged the poor condition of the tank and stated that he would be scheduling the replacement of the tank. I communicated to Mr. Horning that I was setting a deadline of 30 days to have the work completed before escalating enforcement action.
1006439337 2014-12-31 Production Wells No Violations I was on site to conduct a status check. Identification was posted on the tank battery and wellhead. The tank battery consists of one 100 bbl production tank and one horizontal separator. The tank valves are plugged. There is a meter box with a current chart showing production. The dike is present but is in poor condition and needs to be brought up in several places. The tank battery and wellhead are behind a chain-link fence with gates locked. The wellhead is equipped as a plunger lift. The access road and site are stable. The operator will be contacted to address of the dike issues.

1335736530 2016-05-18 Production Wells No Violations I was contacted by The Scenic Vista Park manager Mike West that a tree has fallen on the tank battery. My inspection found a tree has fallen and damaged the fence surrounding the tank battery. The tree needs removed from the tank battery and the fence repaired. The tank battery is on a nature trail for the park. I contacted Pep Horning who takes care of the well and left a message to make the repairs as soon as possible and contact an inspector once the work is complete.
1470317524 2016-06-21 Production Wells No Violations The purpose of my inspection was to follow up on Complaint # 5331. The complainant stated that a tree had fallen onto security fence around a tank battery within the Scenic Vista Park. Inspector Kevin Perry had previously received a call from Park Manager Mike West regarding the fallen tree. My inspection observed that the tree had not been removed as of 6/21/2016. Equipment within the fenced tank battery consisted of a horizontal separator and a 110 bbl storage tank. The access door to the battery was locked, however unauthorized access to the battery could be gained due to damage from the tree. The storage tank load lines were shut and plugged. I.D. signage was on the tank and contained current required information. Corrosion has caused a large hole to develop near the top of the tank. There is rust staining down the side of the tank, but there is no evidence of a brine spill at this time. A separator discharge line at the top of the tank is secured to the tank stairs by means of a nylon strap. I am also concerned with the condition of the earthen dike perimeter of the tank battery. Its current state prevents effective containment of any release within the battery. The wellhead is within a fenced area that is also locked. Minimum distance between the well and tank battery are maintained. The well is tagged with correct information. I observed some corrosion where the tubing sits in the casing hanger, however there was no leakage of gas or fluid at this time. I spoke with Pep Horning at S&S later that morning concerning the time frame to remove the tree as well as to address my other concerns. He indicated that the tree removal and repairs were scheduled for later that week and that production operations had been adjusted to prevent fluid levels reaching the hole observed in the tank side. I also instructed Mr. Horning to communicate to me when the repairs had been made to allow for a follow up inspection.
