0564475088 2016-11-07 Production Wells No Violations The purpose for my inspection was to follow up on action required by the operator from a previous inspection. The Operator has repaired the leak detected at the well head. No further action is necessary.
0586781091 2014-12-31 Production Wells No Violations I was on site to conduct a status check. Identification was posted on the tank battery and wellhead. The tank battery consists of one 100 bbl production tank and one horizontal separator. The tank valves are plugged. There is a meter box with a current chart showing production. The dike is present but is in poor condition and needs to be brought up in several places. The wellhead is equipped as plunger lift. The access road and site are stable. The operator will be contacted to address of the dike issues.
1429291460 2015-04-16 Production Wells No Violations I was onsite to conduct a follow-up inspection. On 12/31/2014 I had conducted a status check where I observed that the dike was low and needed to be raised in the area near the separators. I contacted Pep Horning of S&S Energy, on 1/6/2015 regarding the dike. Upon reinspection, the dike issue remains and has not been resolved.
1477076236 2016-10-18 Production Wells No Violations The purpose of my inspection was to conduct a status check and follow up on a compliance action request documented in Inspection # 1429291460 by Inspector J. Schiering. On April 16, 2015 Inspector Schiering documented that the Operator had not improved the earthen dike containment around the production equipment to sufficiently contain a release of production fluid. During my follow-up inspection, I observed that the earthen dike remained insufficient for containment. A review of reported production indicates the well has not produced any significant volume of fluid since 1996. Access to the well head is through grassland that is stable and in good repair. The well location is within a clearing inside forest that is stable. Correct and legible identification is posted at the well head. The 8.625 casing head is above grade and contains a plug in one side port and a ball valve placed in the closed position on the other side. 4.5 production casing is equipped with a plunger lift assembly, The packing on the tubing slip head is leaking gas. Production valves are in the open position. Production equipment within the dike consists of one (1) horizontal separator and one (1) 100 bbl production tank. Minimum spacing requirements are met. The interior of the separator control cabinet exhibited extensive internal corrosion. The production tank remained in good repair with no leaks. Correct and legible identification was conspicuously placed on the tank body. The load line valve was closed and plugged. The drain line was closed and plugged. The hatch lid was closed. The condition of the earthen dike in relation to the historically de minimis amount of fluid produced does not currently present a clear and present hazard. I spoke with lease operator Pep Horning on 10/24/2016 to communicate the delinquent production and leaking well head. Mr. Horning acknowledged the leak and indicated he would be on location within the week to investigate and repair the
