0123249474 | 2017-02-01 | Production Wells | No Violations | The purpose for my inspection was to conduct a status check on well #2-1805-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road showed signs of channelized erosion but remained stable and drivable. No signs of erosion were observed adjacent to the road traveling to the pad. The soil stockpile was stabilized with well- |

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there is a site drain installed. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and approved.

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The Mudpush spacer was mudscaled at 12.5 pounds per gallon (ppg). Schlumberger pumped 261.7 bbls of lead cement at 13.5 ppg, and 261.6 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 286.5 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 7:06pm on 11/14/2013. The plug landing pressure was 2370 psi, and it was raised to 3343 psi, and held for 5 minutes. Pressure fell slightly to 3309 psi. The float held when the pressure was released. 3.0 bbls of water bled back. Neither slurry nor spacer returned to surface.
0611148086 | 2013-07-23 | Surface Facility Construction | No Violations | I went to the site to review the construction process of the drilling pad. The access road is complete. The road has a crushed limestone surface, and erosion controls are in place. The area of disturbance is seeded and covered with straw. There is a gate with a padlock at the access road entrance. The pad is complete. There is a berm and a chain link fence surrounding the site. There is a padlocked gate in the fence. The area of disturbance and the soil stockpiles have been stabilized. There are two cellars installed at the pad denoted as the Judy Brown 3H and 6H. Each is surrounded by a bull fence to prevent falls. Additionally, two site drains to control precipitation on the pad have been installed. |
0786706718 | 2017-08-30 | Production Wells | No Violations | The purpose of my inspection was to conduct a status check on well #2-1805. The access road, pad and erosion and sedimentation controls were in good repair. The security fence was locked. No signs of vandalism were observed. Correct identification was present. The well head was protected by concrete jersey barriers. The cellar was backfiled with gravel. Annular pressure gauges were plumbed above grade and identified. The 9.625 and 5.5 valves were open. The gauges indicated 0 psi on each valve. No leaks were observed. Production equipment on the pad consisted of a well specific sand knock out and GPU. Common equipment consisted of one (1) hydrate inhibitor tank, one (1) thermal generator, and one (1) meter run and one (1) VRU. The tank battery consisted of three (3) 400 bbl produced water tanks, three (3) 400 bbl condensate tanks. The tank were inside an impervious lined steel containment. Valves were closed and plugged. The containment was holding fresh water. No violations were observed during this inspection. |

1316569684 | 2013-09-05 | Casing String Witnessed | No Violations | I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120. Express installed 120 of 20 OD casing. During drilling, the borehole got damp at an approximate depth of 80, but did not produce much water. Allied Oil & Gas Services was the cementing contractor. The cement slurry was mudscaled at 15.6 pounds per gallon (ppg). Allied pumped 52.4 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39 bbls of water. An estimated 15 bbls of cement slurry returned to surface. Displacement was completed at 1:28pm. |

1406728058 | 2014-05-20 | Production Wells | No Violations | I stopped by the site to check on the status of well completion and restoration. The well has been hydraulically fractured. At this time a work over rig is installing 2-7/8 tubing. Production equipment is on site but not hooked up. I didnot walk the entire site due to the active. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized pending installation of production equipment. |
1813027455 | 2013-09-03 | Drill / Deepen / Reopen | No Violations | I stopped at the drilling pad to check on the conductor rig move-in progress. I found pipe trucks from Express Energy Services on site. Express personnel told me that the rig was expected shortly. |
2143033912 | 2013-04-17 | Surface Facility Construction | No Violations | I went to the site and conducted a pre-construction site inspection with Steve Ault, Chesapeake Field Representative. |
