0000999516 | 2013-07-08 | Drill / Deepen / Reopen | No Violations | I stopped at the drilling pad to check on the conductor rig move-in progress. I found the pad largely built. The chain link fence to surround the pad was not yet installed. The dirt work appeared completed, and erosion prevention measures have been taken. The conductor rig was not yet on location. While there, a truck with conductor casing did arrive. The Pitts 22-14-4 3H and 8H were both staked. I took GPS readings for both wells, and the access road entry. |
0080736728 | 2013-07-14 | Drill / Deepen / Reopen | No Violations | While in the area for a cement job at another location, I stopped by to check on the move-in progress of the drilling rig. I found that preparations were underway, but no rig equipment was on site. Mat boards were down, secondary containment was staged, and a number of frac tanks and light plants were in place. |
0399233534 | 2013-04-17 | Surface Facility Construction | No Violations | I went to the site and conducted a pre-construction site inspection with Steve Ault, Chesapeake Field Representative. |

0795200064 | 2013-07-17 | Drill / Deepen / Reopen | No Violations | I stopped by the site to check on the status of the rigging up of Nomac 74. Most of the major components were on site, but not rigged up. Welding work was being done on the rig substructure before rigging up will be completed. |
1167730018 | 2016-03-29 | Production Wells | No Violations | I was on location for a status check. My inspection found the access road gate locked and the
access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #8H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 6 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 6 - Gas processing units (GPU) 1 - Vertical separator 6 - Sand separator 1 - Thermal electric generator 6 - Producing wells Site fenced and man doors locked |

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The cement slurries were mudscaled to verify the target density. Schlumberger pumped 297 bbls of lead cement at 13.5 ppg, and 301 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 298 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 4:45am on 08/03/2013. The plug landing pressure was 2800 psi, and it was raised to 3615 psi, and held for 5 minutes. Pressure after 5 minutes was 3658 psi. The float held when the pressure was released. 3.5 bbls of water bled back. A total of 55 bbls of the Mudpush spacer returned to surface.
1589286570 | 2017-01-30 | Production Wells | No Violations | The purpose for my inspection was to conduct a status check on well (#2-1814-00- |

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there are two site drains installed. There is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Crops, woodlands and/or underbrush are growing normally outside the area of disturbance. Drainage and erosion controls outside the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary
