0040417213 2013-09-05 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. Express Energy Services drilled a 26 diameter hole to 120. Express installed 120 of 20 OD casing. During drilling, the borehole got damp at an approximate depth of 80, but did not produce much water. Allied Oil & Gas Services was the cementing contractor. The cement slurry was mudscaled at 15.6 pounds per gallon (ppg). Allied pumped 52.4 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39 bbls of water. An estimated 15 bbls of cement slurry returned to surface. Displacement was completed at 11:39am.

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there is a site drain installed. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete and approved.

0166726201 2013-04-17 Surface Facility Construction No Violations I went to the site and conducted a pre-construction site inspection with Steve Ault, Chesapeake Field Representative.
0205390183 2017-02-01 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1823-00-00. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road showed signs of channelized erosion but remained stable and drivable. No signs of erosion were observed adjacent to the road traveling to the pad. The soil stockpile was stabilized with well-
0370995891 2013-07-23 Surface Facility Construction No Violations I went to the site to review the construction process of the drilling pad. The access road is complete. The road has a crushed limestone surface, and erosion controls are in place. The area of disturbance is seeded and covered with straw. There is a gate with a padlock at the access road entrance. The pad is complete. There is a berm and a chain link fence surrounding the site. There is a padlocked gate in the fence. The area of disturbance and the soil stockpiles have been stabilized. There are two cellars installed at the pad denoted as the Judy Brown 3H and 6H. Each is surrounded by a bull fence to prevent falls. Additionally, two site drains to control precipitation on the pad have been installed.

1386083412 2013-11-29 Drill / Deepen / Reopen No Violations I stopped at the location to meet with the drilling consultants regarding the completion of the drilling of the well and the installation of the production casing. The consultants were not at the site. Personnel were actively rigging down and moving to the next location.
1390639990 2013-09-03 Drill / Deepen / Reopen No Violations I stopped at the drilling pad to check on the conductor rig move-in progress. I found pipe trucks from Express Energy Services on site. Express personnel told me that the rig was expected shortly.
1505393799 2017-08-30 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1823. The access road, pad and erosion and sedimentation controls were in good repair. The security fence was locked. No signs of vandalism were observed. Correct identification was present. The well head was protected by concrete jersey barriers. The cellar was backfiled with gravel. Annular pressure gauges were plumbed above grade and identified. The 9.625 and 5.5 valves were open. The gauges indicated 0 psi on each valve. No leaks were observed. Production equipment on the pad consisted of a well specific sand knock out and GPU. Common equipment consisted of one (1) hydrate inhibitor tank, one (1) thermal generator, and one (1) meter run and one (1) VRU. The tank battery consisted of three (3) 400 bbl produced water tanks, three (3) 400 bbl condensate tanks. The tank were inside an impervious lined steel containment. Valves were closed and plugged. The containment was holding fresh water. No violations were observed during this inspection.
1717501436 2013-07-08 Surface Facility Construction No Violations I went to the site to review the construction process of the drilling pad. The access road was constructed, and there was considerable truck traffic hauling rock to the pad. Due the truck traffic, I will check on the pad construction at a later date.
