1003241225 | 2013-10-29 | Drill / Deepen / Reopen | No Violations | I returned to the site in the afternoon of 10/29/2013. Cleanup was continuing. Mr. Shpakoff indicated that once the synthetic oil-based mud (SOBM) was cleaned up, absorbent booms would be situated on the edges of the containment to capture any potential remaining SOBM that would be present. The storage tanks had not yet been removed. I left the site. I was called later in the evening by Mr. Shpakoff, and he indicated that the storage tanks had been moved, and that there was a small amount of SOBM under the containment. The impacted gravel and soil were removed and the pad restored. Mr. Shpakoff emailed images to me showing the work done. |
1014003432 | 2013-11-12 | Completion Testing | No Violations | I was at the location to discuss the drilling of the lateral and to obtain the production casing installation and cementing information and documentation. 1144473381 | 2016-08-23 | Production Wells | No Violations | The purpose of my inspection was to determine the status of preliminary restoration and conduct a status check of the Firestone Homestead Pad B with Regional Emergency Operations Manager Terry Whitehair, and Emergency Operations Coordinator Fred Romeo. Atlas Lease Operator Nelson Case provided entry to the pad and served as a guide for the inspection. Access to the pad is over a lease road that is lined with crushed gravel and remains in good repair. A sign board posted near the entrance identified the operator, well names and permit numbers, and emergency contact information. A chain link fence topped with barbed wire is present around the entire pad. An emergency shut down button is present at the entrance and at all four corner escape doors. I walked the perimeter of the pad and found the exterior perimeter stable with well- established vegetation. The topsoil stockpile is stabilized with well-established vegetation. The surface of the pad was composed of crushed gravel that remained in good repair. Equipment and material not used in the production process have been removed. The Firestone Homestead Pad B currently contains three (3) wells. Firestone Homestead Pad B #3H (API # 2-1828-00-00) is identified with a steel dog tag that contains correct and legible identification. After stimulation, the well was equipped with a Christmas tree valve assembly. The casing pressure gauge indicated 904#. The tubing pressure gauge was inaccessible up on the catwalk. The cellar had plastic pipe installed to facilitate draining and was backfilled with gravel. The mousehole was cemented. No audible or visual leaks were detected. Production equipment on the pad consisted of three (3) gas production units (GPU), having one unit for each respective well, and three (3) 400 bbl production tanks. The GPU contained correct, legible and conspicuous identification and sat adjacent to the units for the other two wells on the pad. All three units were placed on top of conta |
1360947925 | 2014-06-17 | Preliminary Restoration | No Violations | I stopped by the site to check on the status of well completion and restoration. The well has been hydraulically fractured. There is three 400 barrel tanks and three GPU production units. The well is equipped with a Christmas tree. Values are locked out. Tanks or the wellhead do note have ID signs posted. I walked the entire site, and found the location to be in good condition. There is various values and pipe on location that needs to be removed. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized until well is in production and ID sigs are posted and location clear of all material laying on location. |
1379967954 | 2013-08-20 | Surface Facility Construction | No Violations | I went to the site and conducted a pre-construction site inspection with Kirk Elkin and Jim Burns of Atlas. |
1383593008 | 2013-10-18 | Drill / Deepen / Reopen | No Violations | I performed a status check on the above location. I spoke to the drilling foreman and he informed me that they would be running there 9.625 casing and cementing within 16 hours and that they would call me. |
1384440974 | 2013-10-29 | Drill / Deepen / Reopen | No Violations | On 10/28/2013, I received a voicemail from Kirk Elkins of Atlas Noble LLC. He informed me that approximately at approximately 10pm on the night of 10/28/2013, during a transfer from a truck to an on-site storage tank, a hatch on the tank was unsecured, and there was a spill of synthetic oil-based mud (SOBM). He estimated that there was approximately 50 bbls spilled. A portion of the spill escaped containment and spread onto the drilling pad. He indicated that none of the spill escaped the drilling pad, and that cleanup operations were underway. Atlas environmental personnel would be at the site the morning of 10/29. On the morning of 10/29, I went to the site to review the cleanup efforts. I met with Drilling Superintendent Denny Stivanson, and Atlas Regulatory Compliance Specialists Lisa Raffle and Brian Shpakoff. A cleanup crew was at the site excavating impacted pad gravel and underlying stabilized soil. SOBM that was within the containment was removed by vacuum truck. I witnessed that none of the SOBM had escaped the drilling pad. It was estimated that 87 bbls of SOBM were released. 40 bbls were captured by the containment, and 47 bbls spilled onto the drilling pad. When the cleanup outside the containment area is complete, two storage tanks were to be removed so any SOBM that moved under the containment could be cleaned up. I left the site to check on the drilling of conductor holes at another location, and will return later in the day. |
1384444387 | 2013-10-31 | Plug / Plug Back | No Violations | I went to the site to witness a plug--off plug. The operation included setting a single cement plug from the bottom of the hole over the Utica/Point Pleasant. The plug also served as the kick--based mud spill, and found it to be cleaned and restored. |
1415647668 | 2013-10-07 | Drill / Deepen / Reopen | No Violations | I was at the site to check on the rigging up process of the drilling rig. Rig equipment was still being moved in, and rigging up operations were in progress. I met with drilling superintendent Denny Stivanson and discussed the upcoming start of drilling. I set a tentative pre-spud meeting for 10/09/2013. |
1762481504 | 2013-10-19 | Drill / Deepen / Reopen | No Violations | Nomac Rig 323 drilled a 12.25 well bore to a total depth of 2050 and ran 2050 of 9.625 36# J-55 casing with 20 centralizes. Nabors Completion & Production Services were on location to cement the 9.625 casing. They pumped 120 barrels of water follow by 35 barrels of Gel ( 500# ) followed by 5 barrels of water. They pumped 780 sacks of STANDARD TYPE I. 1%cc 1/4# FLAKE. Displaced with 165.8 barrels of water. Landed plug held pressure for 5 minutes floats held. 45 barrels of cement returned to surface. Good circulation throughout job. |
