0105609232 2017-02-01 Production Wells No Violations The purpose for my inspection was to conduct a status check on well #2-1878-00-00. The well is currently not in production. The well has been drilled to total depth but not stimulated at the time of my inspection. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. At the time of my inspection, the local area had received a coating of snow that partially covered the location. The lease road was also utilized for farming and remained drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile was stable with well-

The intermediate and production casing annulars were plumbed above grade and identified. Both valves were open. The intermediate casing annulus pressure indicated 0 psi. The production casing annulus indicated 184 psi. No other leaks were observed. No violations were observed during this inspection.

1148559218 2017-07-14 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1878. The lease access road and pad remained in good repair. I met Chesapeake instrument and electical technician Robby Alexander on location. Contractors were on location installing production instrumentation following stimulation operations. The 13.375 annular pressure gauge was open and indicated 138 psi. The 9.625 annular pressure gauge was open and indicated 340 psi. I will plan to re-inspect the location after well is placed into production.
1255736477 2016-03-31 Drill / Deepen / Reopen No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #6H well is a conductor casing plumbed up and capped. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 2 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 2 - Gas processing units (GPU) 2 - Sand separators 1 - Thermal electric generator 2 - Producing wells

1449684404 2015-11-05 Drill / Deepen / Reopen No Violations Express Energy drilled a 26 hole to a depth of 120 and ran 120 of 20 94 lb./ft. of welded conductor casing. Express Energy was the cementing contractor. They pumped 230 sacks (48.34 bbl..) of class A cement 15.6 lb./gal. The cement was displaced with 39.07 barrels of fresh water. There was 12 barrels of cement returned to surface.
1538865738 2017-08-30 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1878. The access road, pad and erosion and sedimentation controls were in good repair. The security fence remained in good repair. Correct identification was present. The well head was protected by concrete jersey barriers. The cellar was open and contained fresh water. Annular pressure gauges were plumbed above grade and identified. The 9.625 and 5.5 valves were open. The 9.625 annular gauge indicated 0 psi. The 5.5 annular gauge indicated 575 psi. No leaks were observed. Production equipment on the pad consisted of a well specific sand knock out and GPU and a horizontal dehydration unit. Common equipment consisted of one (1) hydrate inhibitor tank, one (1) thermal generator, and one (1) meter run and one (1) VRU. The tank battery consisted of three (3) 400 bbl produced water tanks. The tank were inside an impervious lined steel containment. Valves were closed and plugged. The containment was holding fresh water. I will contact Chesapeake regulatory affairs manager Jody Jones concerning the annular pressure on the 9.625/5.5 gauge.
