Upon arrival I met with company representative Kris Gonzales. Mr. Gonzales accompanied me during the inspection. At the time of inspection, the drilling rig was drilling the 8.75 borehole section on air. A closed loop system solids control system was in operation. No dust was observed on location from the operation. No spills or leaks were observed. During the inspection I discussed the ODNR-DOGRM notification requirements. Mr. Gonzales sent an E-mail on 11/25/2017 reporting that cementing intermediate casing would occur on 11/26/2017. No direct notification occurred. I communicated to Mr. Gonzales that in the future, E-mail notification was sufficient during the course of normal business hours, however during weekends and evenings direct notification would be required to ensure a representative of ODNR-DOGRM could be present to witness operations. No violations were observed during this inspection. |
0519854927 | 2017-01-22 | Production Wells | No Violations | The purpose of my inspection was to witness stimulation activities on well #2-1896. Chesapeake Exploration called ODNR-DOGRM emergency response coordinator Fred Romeo to give notice that stimulation work was to commence. Identification posted at the entrance indicated the incorrect township and county. Halliburton was the pressure pumping contractor. Fluid handling equipment was located on containment. No leaks were observed. At the time of my inspection stage 2 of 74 was being pumped in well 2-1895 while 9 Energy Services perforated the next stage in well #2-1896. The 5.5 production casing annular pressure gauge indicated 43 psi. While on location I met with Chesapeake representative Ryan Boggs to discuss notification. Mr. Boggs stated that job binder issued to the location was not updated from the office. While on location I called and spoke with Chesapeake Regulatory Manager Jody Jones regarding the location identification and notification. Mr. Jones stated that he would have corrected identification ordered and posted. He continued that he would discuss updating the contacts list internally with the responsible party and reach out to ODNR-DOGRM for clarification. I will schedule to re-inspect the location prior to conclusion of stimulation work. |

I walked the area of operation all contianment was in place. No violations were noted at this time. |

Contractors were on location installing containment and setting up the backyard in conjunction with moving the bottom hole rig onto location. Drill cuttings from the conductor section were being loaded onto a truck for disposal. No violations were observed during this inspection. |

I met Chesapeake representative and location supervisor Bryan Waid on location. Flowback operations for well 2-1896 were occurring at the time of inspection. Correct identification was present at the well head. The intermediate and production casing annular gauges remained labeled and plumbed above grade. Valves were in the open position. Each gauge indicated 0 psi. No evidence of leaks were observed. Recovered fluid was transferred to steel 500 barel tanks that were positioned inside of containment. I did not witness any evidence of leaks. No violations were observed during this inspection. |

I arrived on location and observed Wyoming Casing installing new 5.5 P-110 23 lb./ft. casing into the well. I walked the location and did not witness any evidence of spills or leaks off of containment. No odors were detected. I met with company representative Will Doby to discuss the cement plan. Mr. Doby stated that O-Tex pumping would be the production casing cement contractor. At the time of inspection Mr. Doby did not have a formal pump schedule for the job, but stated the plan called to cement with 15.6 ppg Class H cement that would be circulated back to surface. No violations were observed during this inspection. |

The well cellar was covered with steel grating to prevent falls. I was unable to document the status of the mousehole due to ice and snow on the surface. No leaks were observed. No violations were observed during this inspection. |

Mr. Waid reported the newly completed wells were being monitored due to sanding issues at surface. At the time of my inspection the 9.625 annular valve was open and the installed gauge indicated 0 psi. The 5.5 annular valve was open and the installed gauge indicated 0 psi. The cellar remained open and contained meteoric water. No sheen was observed. No audible or visual leaks were observed. The mousehole was cemented. While on location a Stallion vacuum truck arrived on location and was loading production fluid from the tank battery. I did not observe any leaks, spills or malodors. No violations were observed during this inspection. |

The cement bond log indicated acceptable bonding in the entire annular space from casing seat to surface. While on location I met with drilling consultant Danny Lambert and discussed notification requirements. No violations were observed during this inspection. |
