1496417219 2017-06-01 Production Wells Violations Noted The purpose of my inspection was to conduct a status check of well #6-1783. A review of RBDMS indicated zero production in the previous two years. Access to the well was through a farm field road that was stable and drivable. Surface owner Matt Boyce accompanied me during the inspection. The well was located within an agricultural field and was stable. No identification was present on the well. I hung flagging with the permit number, well name and owner for future identification for later surveying by ODNR-DOGRM. The 5.5 production casing was equipped with a well head and fitted with 2.5 tubing. The well head was open to atmosphere. No audible or visual leaks were detected. No pumping unit or flow line was connected to the well to enable production. I spoke with well owner Shawn Watson who stated he was working on posting identification at the well head. The tank battery consists of one (1) 40 bbl production tank and one (1) concrete brine vault. The earthen dike constructed around the tank and vault was worn. The tank remained in good repair. No holes or evidence of leaks was detected. The tank was emptied of fluid. The identification on the tank body was illegible. The load line valve was closed and plugged. The concrete brine vault was holding water at freeboard. No evidence of overflow was present. The well was currently incapable of production. Shawn Watson shall submit a written plugging schedule to Inspector Ken Frye at the Ohio Department of Natural Resources, Division of Oil and Gas Resources Management, 3575 Forest Lake Drive, Suite 150, Uniontown, OH 44685, within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. Shawn Watson will be notified if the plugging schedule is approved or denied.
1569667899 2019-01-17 Production Wells No Violations The purpose of my inspection was to follow-up on compliance notice #1496417219. RBDMS indicated the Owner reported zero production in 2017. The location remained stable. No identification was present. No audible or visual leaks were observed. The tank battery consisted of three (3) stock tanks. The tanks remained in good repair. I did not observe any holes or leaks. Load out valves were closed and plugged. The hatch lids were closed. I spoke with Shawn Watson on 01/25/2019 regarding the identification and well head condition. Mr. Watson reported posting identification at the well and stated he would revisit the location to replace it. He continued that he was working to finance plug and abandonment on the well.
