0089492785 2018-09-19 Field Inspected, Well Not Found No Violations The purpose of my inspection was to attempt to field locate well #6-2759. Through E-mail I received a request to locate well #6-2759 from Dave Bodo Jr. with Bodo Surveyors. Mr. Bodo was working for the surface owner in efforts to construct a new building and the concern was the new building would be locate near or overtop of the plugged well. While on location I met with operations manager Don Wilson. During my field investigation I was not able to find evidence of the former location. Mr. Wilson and I located the last coordinates and he planned to have an employee excavate the area to locate the well. If unable to locate the well, Mr. Bodo would be on location on 9/21/2018 and would make another attempt with survey equipment. Once located, I will schedule a follow-up inspection to document the status of the well.
1124204717 2004-08-16 Orphan No Violations Arrived on site this date to work with the company. The well is an abandoned leaking well. Leaking gas and oil with an obstruction at a depth of approximately 45. The well was baled down and hole plug bentonite clay was mixed with water back to the surface. There was no further leakage observed. The company is to cut the well off to facilitate mowing.
1537364864 2018-09-19 Field Inspected, Well Not Found No Violations The purpose of my inspection was to assist in field locating well #6-2759. The surface landowner has plans to construct a building close to the plugged and abandoned well. I was contacted by David Bodos Surveying concerning the well and location. At the time of my inspection I met with Don Wilson, Supervisor with Magneco-Metrel. I located the well coordinates listed in RBDMS and Mr. Wilson marked a perimeter of ten (10) feet around the location. Mr. Wilson planned to attempt to excavate and uncover the former well location with the assistance of the David Bodos Surveying and a metal detector. I will communicate with ODNR-DOGRM Surveying Manager Robert Lowe and Emergency Response Section Coordinator Fred Romeo to assist in documenting the location and current status of the plugged well. I will schedule a follow-up inspection of the daylighted well head.
1537812635 2018-09-24 Plug / Plug Back No Violations The purpose of my inspection was to witness the status of well #6-2759. The survey firm of David Bodos uncovered the well head on 9/21/2018. Location coordinates from Bodos surveying are in NAD 83. On 9/24/2018, I met with Magneco-Metrel facility manager Don Wilson and ODNR-DOGRM Emergency Response Manager Andrew Adgate and Coordinator Fred Romeo to conduct and inspection of the well at surface and utilize a forward looking infrared (FLIR) camera to determine if the any stray gas was leaking from the well head at surface. At the time of my inspection I did not observe any evidence of gas or oil leaking from the well head. Fred Romeo did not observe any evidence of stray gas with the FLIR camera. According to Mr. Wilson, the plugged well location will be within the area planned for an expansion building. Mr. Wilson stated that he would plan to have the area over the well sectioned from the other floor if later intervention were required. I suggested that during construction a vault be placed around the well head and a vent pipe lead away from the vault to the outside of the building. Mr. Wilson agreed and planned to discuss the design with the company architect and engineer.
