2062710517 2018-08-08 Orphan No Violations I met with City of East Liverpool Assistant Safety Director Rick Rudibaugh on location to follow-up on complaint # 5926. No identification was present. The well was not registered with ODNR-DOGRM and did not have an API number. At the time of inspection, an orange traffic cone was placed over the casing/well head to prevent an impact. No other identification was present at the well head. I observed 5.5 O.D. casing above grade. The casing was fitted with a 5x2 swage that was open to atmosphere. I did not observe any fluid or gas flow from the well during the inspection, or previous evidence of flow. No other equipment was on location. Mr. Rudibaugh stated that no one could remember the well being operated. I will follow-up with an idle well report.
