0162129944 2019-05-06 Final Restoration No Violations On the day of my inspection I found that all oilfield related materials and debris had been removed from the site. There was no production equipment associated with this well. The well site and access road were in an agricultural field. The field had been plowed, planted and was growing in winter wheat. There were no issues with erosion at the site. This site passes final restoration.
1495625863 2018-10-23 Plug / Plug Back No Violations On the day of my inspection I met Brian Darr at the well site to witness the casing being cut and capped. Brian cut the well casing off approximately 40 inches below grade and capped it with a steel plate that was inscribed with the permit number. Brain said that he would have all of the remaining clay and the pulled casing out of the site by the end of the day. The well was located in a corn field and it will be planted in the spring.
1540218360 2018-10-19 Plug / Plug Back No Violations On the day of my inspection I visited the well site with DOGRM Surveyor Tom Smith to record the well location. I found that the well casing was still open and the clay was down approximately 10 feet. I dropped a clump of clay in and the column was solid. The casing and tubing that had been pulled from the well was still laying on the ground. Phil Moran had told me prior that the landowners were going to cut and cap the well, and they were going to remove the pulled casing. I called Brian Darr and he said that he would be out next week to cut the casing off and put the steel plate on. There is a possibility that this well was plugged and abandoned without the authority of the Ohio Division of Oil and Gas. If that is the case then there is also a possibility that the surface and conductor casing were pulled from the ground when this well was plugged and that is the reason why we could not located the well with the magnetic finder.

1878964216 2018-10-15 Plug / Plug Back No Violations On the day of my inspection I found that the well was filled with clay to within 5 feet of the surface. The conductor casing was still open and the surface casing was stacked on the ground. I called Phil Moran to ask about the job. He said that he got 285 feet of the 5 inch diameter casing out and he got approximately 6 tons of clay in the well to fill it to surface. Phil said that the well owner was going to haul the pulled casing away once the corn was off. He also said that the well owner was going to cut the conductor casing and cap it with a steel plate. I contacted inspector Randy Akins and he said that he had a copy of the clay ticket.
