0051849831 2017-06-20 Production Wells No Violations On the day of my inspection I found that the old 100 barrel tank was still lying on its side, and the oily gravel was still in place where the tank was leaking. The site was starting to get taken over by weeds again. I called James Vokoun to ask what they are doing. He claimed that they were waiting on a new tank. I told him that I was concerned with getting the oily gravel cleaned up, and they can handle the tank change on their own time. I stressed that they need to get moving on the clean up and get it done as soon as possible. I told him that I would be back by in a couple weeks.

1132567576 2017-04-04 Administrative Inspection No Violations I received a phone call from James Vokoun Jr. on behalf of his father, the owner of the well. He said that they have started cleaning up the brush and removing some of the contaminated soil. He said that they were getting a water truck to finish draining the tank and then they were going to lay the tank on its side to assess the possibility of repairing the tank or having it turned. He said that they would finish removing any contaminated soil when the tank is moved. I told Mr. Vokoun that I would be out periodically to check on their progress.
1249521979 2017-04-21 Production Wells No Violations On the day of my inspection I found that the 100 barrel steel tank had been laid on its side. There were three small holes in the bottom. The oil stained soil and gravel was still in place.
1389911724 2017-08-07 Production Wells No Violations On the day of my inspection I found that the oily soil and gravel had been removed. The old 100 barrel steel tank had also been removed, and the excessive vegetation had been cut down. I found no oil contamination in the soil around the area where the production equipment had been set. Mr. Vokoun still needs to post well identification to resolve the compliance notice.
1444179757 2017-07-27 Production Wells No Violations On the day of my inspection I found that nothing had ben done since my last inspection. The old tank was still laying on its side. The oil soaked gravel and soil was still in place where the tank used to sit. The entire site was becoming overgrown with vegetation. I called the number in RBDMS and I did not get an answer. I searched the internet for an alternate phone number with no luck. James Vokoun Jr. happened to drive by while I was sitting at the site. He stopped to talk and give excuses as to why the clean-up is till incomplete. I stressed the need to address the oil right away and reminded him that the compliance notice is long overdue. I also recommended that he do something about the vegetation before it gets out of hand. He claimed that he has a new tank lined up and he has someone lined up to excavate the impacted soil. He also claimed that he would get something done this weekend. I told him that they need to show some progress. I got Mr. Vokouns cell number and I told him that I would be in touch to follow up.
1491410229 2017-04-05 Production Wells No Violations On the day of my inspection I met with James Vokoun at the tank battery site. The water truck was leaving from draining the tank as I pulled up. The brush and small trees had been cleared from around the production equipment. Most of the impacted soil around the tank had been cleaned up. Mr. Vokoun said that they were going to lay the tank down on its side to get the remaining soil underneath. While the tank is on its side they will assess the condition and make the decision to either repair the tank or remove it. Mr. Vokoun said that the well doesnt make much fluid so I suggested going to a smaller poly tank.
1527292896 2017-03-15 Production Wells Violations Noted On the day of my inspection I found that the well was equipped with an electric powered pumping unit. The motor and drive belts were intact. The electric was on in the breaker box and cable was connected to the motor. The polished rod showed signs of use. The casing valves were open and connected to plastic gas lines. There were no visible leaks at the wellhead. The tank battery consisted of one 100 barrel steel tank in an earthen dike, one 20 barrel poly tank in a separate shallow dike, and one vertical separator outside of the dike. The production equipment was heavily overgrown with vegetation, and the 100 barrel tank was rusted. There was no identification posted at the wellhead or tank battery. I found a 2 foot area of crude oil saturated soil and gravel at the base of the tank. If the tank is leaking it must be repaired or replaced. The contaminated soil and gravel must be removed from the dike. Correct, legible identification must be posted on the tank to include well owner, lease name and well number, and emergency contact phone number. It is also recommended that methods be employed to control the excessive vegetation around the production equipment. The above mentioned compliance issues should be resolved within 30 days of the issuance of this notice.
1714854591 2017-08-25 Production Wells No Violations On the day of my inspection I found that the separator was still in place near SR 206 and the old tank had been moved back near the wellhead. I spoke with James Vokoun to remind him that he needs to get identification posted for the well to resolve the compliance notice. We also discussed eliminating the production equipment along the state route and setting the new tank back near the wellhead. The access road is in good condition and relatively flat. I told James that he just needs to make sure the tank is at least 50 feet from the wellhead. He said that he would get the ID posted and give me a call.
