0183883762 2016-09-06 Production Wells No Violations I received a phone call from Chuck Donley of TLC Oil to notify me that he had addressed all of the issues with the wells on this lease. On the day of my inspection I found that the 55 gallon drum had been removed from the tank battery. The oil stained soil and gravel had been removed. There were no father issues with contamination at the tank battery.
0292776358 2016-08-03 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped with a natural gas powered pumping unit. The engine and drive belts were intact. The flow lines were connected. The tubing and casing valves were open. I found no leaks or spills around the wellhead. Correct, legible identification was posted at the wellhead. The tank battery consisted of one 210 barrel steel tank and one horizontal separator. The gauge on the separator showed 100 psi. There were two drip bottles and house gas meters at the separator. The valves were open to the Meade property; the valves were closed to the Gibson property. There was a 55 gallon steel drum next to the drips that was split open at the bottom and leaking oil onto the ground. The oil impacted area was 1 x 5 running down slope toward the tank. There was no dike around the production equipment. The tank battery is shared with six other wells on the Stuller lease. Correct, legible identification was posted on the separator. I called Chuck Donley of TLC Oil. He claimed that he would be out to get the oil cleaned up and to remove the drum at the tank battery. He also claimed that they are waiting on Maram Energy to come out and set a new tank battery for the wells before putting them into production. Chuck said that the bulk of the lease is too far from the existing tank and they wanted to set tanks closer.
0710780502 2017-03-31 Production Wells No Violations On the day of my inspection I visited the site of the flow line leak with DOGRM Emergency Response supervisor Trisha Cartmell. We found that the line had been repaired and the impacted soil at the point of the leak had been removed. It was raining and the stream was running fast with a heavy flow. We walked the stream to the main boom/pad collection point. All of the absorbent materials had been cleaned up and there were no signs of residual oil contamination throughout the section of stream that was walked.
0756354293 2017-03-25 Production Wells No Violations I received a phone call from Rob McMasters with the Coshocton County EMA to notify me of a flow line leak. He said that the Walhonding Valley Fire Department and Ohio EPA had also been notified. The leak was discovered by the neighboring home owners on 3/24/17 at 20:30. They allegedly called the number on a line marker at 08:30 on 3/25/17. The homeowner said that he was told that the lease was sold three years ago and was given no more information. The home owners did not report the incident to the EMA until 13:00 on 3/25/17, and I was notified at 14:50. When I arrived at the location I was told that the Ohio EPA would not be responding to the incident, although they were notified. When I walked back to the leak I found that there were multiple steel two inch lines crossing a stream. There was a 10 inch diameter hole dug out at the point of the leak, just along the stream bank. The hole was full of water and there was constant bubbling of natural gas in the water. There was a small skim of crude oil on the water in the hole, and there was a 2 foot area of oil stained soil around the hole. The Walhonding Valley Fire Department had placed absorbent pads and booms at intermittent locations along the stream. The final collection point was approximately 150 feet downstream at a natural dam of leaves and woody debris. There was a slight sheen on the water, but I found no crude oil concentrations except where the final boom/pad collection point was. There were fresh pads down and a garbage bag of used pads. I estimated that less than 10 gallons of crude oil got into the stream. There were no conspicuous line markers near the stream crossing. The marker at the top of the driveway, near SR 541 was labeled ASNF. All of the producing wells in Coshocton County under ASNF were transferred to KMFAD in 2014. My well map of the area showed two KMFAD leases; the Stuller wells to the north and the Kaple wells to the south. Those two leases had recently transferred to TLC Oil & Gas, w

1457032038 2016-03-01 Production Wells No Violations I received a call from Carrie Moore claiming that she had sold the wells on the Stuller lease to TLC Oil and Gas, owned by Ted Morris, Les Donley and Chuck Donley. She was concerned that they were servicing the wells and leaving them in a state of disrepair although they had never taken the proper steps to get registered and bonded with the State of Ohio. On the day of my inspection I found that the well was being produced with a natural gas powered pumping unit. The tubing and casing valves were open. The engine belts were intact and it was connected to a gas line from the casing. Identification was posted at the wellhead, although it still reflected KMFAD as the owner. I found no leaks at the wellhead. The tank battery consisted of one 210 barrel steel tank and one horizontal separator. The gauge on the separator showed 80 psi. There were two house gas meter connections and drip bottles at the separator. There was no containment dike around the production equipment. The well owner and contact number had been changed on the identification sign at the tank battery to reflect TLC as the well owner. I advised Carrie Moore that it was her responsibility as the transferor to submit the change of ownership form 7 within 30 days of the date of transfer per ORC 1509.31. I contacted Cynthia Marshall at DOGRM Central Office and found that she had no record of TLC Oil and Gas applying for an operator number. I contacted Ted Morris to advise him that they must cease all activity until they get registered and bonded with the State of Ohio.
