I have tried to call Carrie multiple times over the past two weeks and have not been able to get an answer or return call. I did get Mike Ingalls to return my call. He said that he was up there yesterday and had the well pumping briefly to check the flow line for leaks. Mike said that he would be out today to remove the old electric line and to start pumping the well. We are scheduled to meet on Monday 11/14/16 to witness the well producing. |

I requested that she make this well a top priority and that she keep an open line of communication with me. |

She claimed that they were seeing some gas pressure after the first microbial treatment, so they wanted to treat again. After the second treatment the well would set for another 21 to 30 days; at which time a service rig would be brought in to pull the rods and tubing and service the downhole pump. Then the well would theoretically be ready to go back into production. I asked her to provide me with a written plan of action before I would grant the extension. |

On the day of my inspection I found that Mike Ingalls was at the well site. He claimed that they have seen no results from the microbial treatments, so he treated the well with 100 gallons of solvent in the morning. The electric lines and pole had been removed, and Mike had a trailer mounted welding unit next to the well to provide power to the electric motor. He did not start the motor or pump the well. The valves were all closed, and the tubing gauge showed 15 psi., but Mike said that it was not making any fluid. He said that they had Mr. Slaughter connected for house gas at the separator and claimed that he was getting gas. I told Mike that they have had more than enough time to get this well into production. There will be no more extensions. I also told him that he needs to show production from the well; just showing that the pumping unit can operate is not sufficient. Mike said that he would be out in 24 hours to check it again. The well is still idle and not in production. The violation is not resolved at this time. |

There were no signs of any recent vehicle or foot traffic to the well. The pumping unit was still in place and the lines were all connected at the wellhead. All of the valves were closed on the tubing and casing nipples. The pumping unit was equipped with an electric motor, but the electric line and box had been removed. This well was required to be plugged and abandoned by 6/8/15 and there were multiple extensions granted with the latest resolve by date being 11/1/16. Nothing has been done recently to put this well back into production, and there has been no application submitted for a permit to plug this well. Carrie Moore has been unresponsive to my attempts at communication today. |

Kate Hornbrook, petroleum engineer with Locus claimed that the well would need to be shut in for seven days to allow the microbes to colonize. She claimed that the microbes would consume the paraffin and open the porosity of the formation. After seven days they could begin to pump the well. It may take a few weeks to see any results. The pumping unit was equipped with a new gasoline engine. I will follow up in one month. KMFAD has an extension date of 9/8/2016 for the compliance notice. |

The solvent drip bottle and belt shroud were laying on the ground. There were no leaks around the wellhead. The production equipment for this well had been removed. There were no vehicle tracks back to the well, and no signs of any recent work at the well site. The landowner claimed that this well had not produced in over 15 years. There has been zero oil or gas production reported for this well since 2014. This well has an outstanding compliance notice to be plugged, with a remedy date of 11/1/2016. I took updated photographs of the pumping unit and production equipment site. |

On the day of my inspection I found that the wellhead was equipped with a pumping unit. The unit was equipped with an electric motor. The electric was disconnected at TR 59 and it was disconnected at the electric box at the well. The electric line was had fallen from one of the poles and was laying on the ground. I observed a cow rubbing against the line, indicating that the line was not transmitting current. The polished rod was dry. The tubing valve was closed and the casing valves were open. The tank battery consisted of one separator. There was no identification posted for the well and I found no leaks or spills. I spoke with Mr. Slaughter. He claimed that the well had been idle for 15 years and the storage tank was removed approximately 10 years ago. He also claimed that he has not received house gas from this well in approximately 15 years. He admitted that his gas was coming from the connection at the Slaughter 2 tank battery on his brothers property that was being back fed from the main sales line. He added that he has not received any house gas since the winter of 2013. In its current state this well is not capable of commercial production. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
