0303177169 2014-08-28 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed 9 tons of clay down the well filling it to surface. I requested that they leave the well open to allow the clay to settle overnight.
0611509520 2014-08-29 Plug / Plug Back No Violations On the day of my inspection I found that the clay was sitting firm at the surface of the well. I authorized Patten Drilling to cut the casing and cap the well.
0913832734 2015-06-22 Final Restoration No Violations On the day of my inspection I found that the well site and lease road were growing with well established tall grasses. I found no signs of erosion. This site passes final restoration.
1295886457 2011-01-24 Production Wells No Violations Last production reported was in 2001. Talked to Bill Patten by phone, said the company was planning on plugging well this summer and would apply for a plugging permit.
1408976206 2014-08-25 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny had pulled the 4.5 casing from the well. They set a brush and stone bridge at 1092 and then made an attempt at pulling the 7 casing. The casing parted as soon as they stared to pull. They got 117 of 7 casing out of the well. The casing was rotted and full of holes at the point where it parted. Matheny washed 5 tons of clay down the well. The well did not fill up. It will be allowed to settle overnight before adding more clay.
2095648115 2016-10-12 Final Restoration No Violations On the day of my inspection I escorted surveyor Tom Smith to the plugged well site in order to mark the location with a flagged stake at the final recorded GPS coordinates. I found that the vegetation was well established and there were no signs of erosion. There was no evidence that the plugged well was leaking.
2109462230 2015-05-05 Final Restoration No Violations On the day of my inspection I found that the site had been graded out and seeded. There was about 60% grass coverage. Some spots were growing well, others were patchy. All production equipment, and oilfield related materials and debris had been removed from the site. I called Billy Patten to request that he re-seed the site. This site does not pass final restoration at this time.
2130851168 2014-08-21 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed 2.5 tons of clay down the 4.5 casing. They ripped the casing at approximately 2000 and pulled two joints. Then they washed another 2.5 tons of clay down the casing for a total of 5 tons of clay for the bottom plug.
