0375403494 2015-07-29 Production Wells No Violations On the day of my inspection I visited the well site with SE Regional Supervisor Brent Bear. We found that the wellhead was still equipped with a pumping unit. The lines at the wellhead were still disconnected. The pulley belts on the pumping unit motor were slack. The cap on the gearbox was open. The polished rod showed no signs of recent use and the bridle was rusted. The unit was overgrown with vegetation. We found 1 plastic line leading from a regulator at the wellhead into the forest in the direction of John Richards farm. The lines at the tank battery were connected. There were no leaks or spills. I took new photos of the disconnected lines at the wellhead.
0601874437 2014-04-14 Production Wells No Violations FOLLOW UP TO CA #-773610376: I have received no further correspondence from Prime Time Energy in regards to the status of this well. The pumping unit does no show any evidence of use. The belts are still slack and the polished rod is still dry. All of the lines from the casing and tubing are disconnected. In its current state this well is not capable of production. I attempted to make contact with the company again. There was no answer so I left another voice message.
0773610376 2013-04-18 Production Wells Violations Noted On the day of my inspection I found that the well head was equipped with a pumping unit that was overgrown with vines and small trees. There were no belts on the pulley, no motor and the gear box was full of water. In its current state this well is incapable of production. This well shares a tank battery with permit 3941; however, no identification was posted at the well head or tank battery for this permit. Make well capable of commercial production within 30 days; or plugging is required within 90 days. Failure to comply may result in forfeiture of bond.
0858352551 2014-08-13 Production Wells No Violations On the day of my inspection I found that the pumping unit still did not show any evidence of use. The belts were still slack and the polished rod was still dry. I took photographs to show that all of the lines from the casing and tubing were still disconnected.
1024392469 2017-10-25 Production Wells No Violations On the day of my inspection I found that the well was equipped with a natural gas powered pumping unit. The unit was moderately overgrown with vegetation and the polished rod showed no indication of recent use. There was a plug screwed into the top of the gear box. The drive belts were tightened, but the fuel line to the engine was still disconnected. Both tubing valves were plugged and one of the casing valves was plugged. The second casing nipple valve was open and connected to 1 inch line, through a regulator and into T fitting. The gauge on the line prior to the regulator showed 200 pounds. From the T fitting the line split to the disconnected engine fuel line and through a valve to plastic line that ran through the woods to permit 3941. The valve for the alleged house gas line was closed. I could hear the hiss of gas escaping from the packing around the tubing in the wellhead. Identification was posted on the pumping unit. Prime Time Energy is reporting over 300 barrels of oil for this well in 2016, even though there is no evidence that the well has been pumped in years. The reported production identically matches the reported production for permit 3941. Although this well is holding gas pressure, there are no instruments in place to measure gas volume in order to quantify the wells capability for sustained gas production. John Richards came out to the storage tanks while I was sitting there writing my report. He claimed that he had a gas leak at a plastic fitting on the woods, and that was the reason the valve was closed at the wellhead. John Maintained that he heats his home and he heats the tanks with the gas off of this well. I suggested to John that he install a gas meter at the wellhead to measure gas use. I told him that I would contact Prime Time Energy with the suggestion.
1410502568 2015-09-04 Production Wells No Violations On the day of my inspection I was approached by the landowner, John Richards. He claimed that he was getting house gas from this well. He also confirmed that the 1 plastic line was tied in with permit 3941. He informed me that Prime Time Energy was offering the well to him for use as an exempt domestic well.
1421075561 2015-01-12 Production Wells No Violations On the day of my inspection I found that there had been no change in the condition of this well. The lines at the wellhead were disconnected. The pulley belts on the pumping unit motor were slack. The polished rod showed no signs of recent use.
1463406315 2016-05-12 Production Wells No Violations On the day of my inspection I was accompanied by Southeast Region District Supervisor Mike Moreland. We found that the tubing lines were both still disconnected. One of the lines off of the casing was disconnected and the other was equipped with a relief valve, a gauge and a ball valve into 1 plastic line. The line was run into the forest in the direction of the Richards #2. The valve to the 1 line was closed. There was no fuel source connected to the engine. The dump valve at the separator was frozen shut. We followed the 1 line through the forest and found that it was connected to a T fitting that tied in with a 1 plastic line from the Richards #2. The valve at the T fitting was closed. The phone number for Prime Time Energy was no longer in service. I called Eric Ashcraft to ask about plugging the well and to get an updated phone number. I informed him that the well was under an order to be plugged. He said that his aunt and Uncle (Denny and Sharon Zolman) were out of the country, but he would notify them when they returned. Eric also called back to tell me that he had spoken with Randy Moran about plugging the well and Moran said that he was backed up with work for 2 months.
1463494746 2016-05-17 Administrative Inspection No Violations I was able to contact Sharon Zolman of Prime Time Energy with the phone number provided by Eric Ashcraft. I spoke with Mrs. Zolman about the current state of the well and the Chiefs Order that required the well to be plugged by June 13, 2015. Mrs. Zolman claimed that she was in agreement that the well needs to be plugged as soon as possible and she claimed that she would get her application for a plugging permit in right away. I emailed all of the pertinent information and forms needed to start the application process.
1477414088 2016-10-25 Production Wells No Violations On the day of my inspection I found that the well was still equipped with a natural gas powered pumping unit. The engine and drive belts were in place, but there was no gas line or external fuel source to the engine. The stuffing box and polished rod were both dry, and the polished rod was crooked as it came out of the stuffing box. The tubing valves were closed and disconnected from the flow line. The ends of the tubing valve nipples were equipped with bull plugs. One of the casing valves was also disconnected from the flow line and capped with a bull plug. The other casing nipple valve was open and connected to a regulator that ran to a T fitting. The T fitting was connected to 1 inch plastic lines with separate valves. The valve to the engine gas line was closed and the line was laying on the ground, open ended. The valve to the other plastic line was open. The line fitting was equipped with a new gauge, which showed 200 pounds. The line ran into the forest in the direction of the number 2 well. There was another T fitting back at the number 2 well, where plastic lines from both wells came together and connected to a third line that ran toward the Richards home. The valve from the number 1 well line to the T fitting was closed. There was no identification posted at the wellhead. I found no leaks or spills at the wellhead. The tank battery is shared with the number 2 well, permit 3941, and consisted of three 100 barrel steel tanks and two vertical separators in an earthen dike. Correct, legible identification was posted on the tanks for both wells. I found no leaks or spills around the production equipment. I spoke with John Richards, and he maintained that the well did not have a hole in the casing. He said that he thinks the water that Guy Dugans well is picking up is coming from an old Pure Oil well that was in the same field and plugged years ago. Mr. Richards said that the well has enough gas volume to heat the tanks, and to provide domestic house gas. This w
1529404490 2014-01-17 Production Wells No Violations FOLLOW UP TO CA #-773610376: On the day of my inspection I found that a motor had been installed on the pump jack. The pulley belts were slack, the polished rod was dry and the bridle was rusted. There was 1 plastic line from permit 3941 connected to this well. Although a motor has been installed, the pump jack does not demonstrate evidence of use. The production records are exactly the same for this well as the Richards 2. The tank battery was freshly painted and identification was posted. I called Primetime Energy and left a voice message.

1959952369 2016-07-19 Administrative Inspection No Violations I called Sharon. Zolman of Prime Time Energy to ask about their proposed time frame in regards to plugging the well. She claimed that she was working with her attorney to submit a request to DOGRM central office to keep the well. Mrs. Zolman maintained that the well was capable of producing house gas and gas to heat the tanks at the tank battery. She said that they were out recently to equip the well with a new gauge and it showed 210 lbs. of gas pressure.
