0041680428 | 2015-12-04 | Production Wells | No Violations | On the day of my inspection I met with the landowner John Slaughter while searching for the Slaughter 1 well. He claimed that the Slaughter 2 well had been idle for the entire year of 2015. He claimed that the operator put in a new flow line in late 2014 and they shipped a tank of fluid from the well in April of 2015. Mr. Slaughter said that he has not observed any activity at the well and he has not observed the pumping unit operate in the past year. Additionally, Mr. Slaughter claimed that the well owner told him that there was a hole in the casing. Finally, he said that he has not received house gas from this well for 15 years. He claimed that the gas he was receiving came from the main line back feeding into the tank battery, and that gas stopped after the winter of 2013. I found that the wellhead was equipped with a pumping unit. The combustion engine was intact. The polished rod was dry. The tubing valve was open and the casing valves were closed. The tank battery consisted of one 100 barrel tank and one separator. The identification was updated at the wellhead, but it was wrong at the tank battery. I found no leaks or spills. I called Carrie Moore to question her about the status of the well. She claimed that she was aware of the situation and that she had intentions of addressing the well after the first of the year. I told her that she might want to look into temporary inactive status if she believes the well could be made capable of production within the next year. |
0786533880 | 2019-04-16 | Production Wells | No Violations | On the day of my inspection I was accompanied to the well site by SE Regional Supervisor Brent Bear. We found that the well was equipped with a natural gas powered pumping unit. The engine, drive belts and fuel line were intact, but the spark plug wire was disconnected. The flow lines were all connected at the wellhead. The tubing and casing valves were all closed. The solvent drip bottle was laying on the ground. There were no leaks around the wellhead. The 100 barrel tank had been removed. The production equipment consisted of one horizontal separator, and a series of domestic gas equipment. The gauge on the separator showed zero pounds. The landowner claimed that his well has not been produced since 2014. There has been zero oil or gas production reported for this well since 2016. This well has an outstanding compliance notice to be plugged, with a remedy date of 3/3/2018. I took updated photographs of the pumping unit and production equipment site. |
1504194638 | 2017-08-31 | Production Wells | Violations Noted | I received a phone call from landowner John Slaughter to report that the Slaughter #2 well has been idle for close to three years. The well was equipped with a natural gas powered pumping unit. The engine, drive belts and fuel line were intact. The flow lines were connected, but all of the valves were closed. The polished rod and stuffing box were dry, and the pumping unit was moderately overgrown with vegetation. Correct, legible identification was posted on the belt/pulley shroud. The production equipment consisted of one 100 barrel steel tank and one horizontal separator. The gauge on the separator showed (0) zero psi. and the tank was empty. The identification sign on the separator had not been updated since the well changed ownership in 2014. I first met with Mr. Slaughter in 2015 while searching for the #1 well, and at that time he claimed that this well had been idle for at least a year. He also said that he does not receive any domestic gas from this well, and he is not receiving any royalties. Mr. Slaughter claimed that there has been no activity at this well since our last meeting two years ago. There were 15 barrels of oil reported in 2015 and no reports have been filed for 2016.While the well is equipped for production it is currently idle and has been idle for at least two years. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |
1817932606 | 2018-06-01 | Production Wells | No Violations | Carrie Moore contacted me earlier this week to notify me that she would be out today with a propane tank to start the well, pump the fluid off and assess the production capability. On the day of my inspection I found that no one was here working on the well and there were no changes to the status of the well. The portion of the access road that leads from Mr. Slaughters farm road to the wellhead was overgrown with waist high grass. There were no signs of any recent vehicle or foot traffic to the well. The pumping unit was in a state of abandonment and all of the valves at the wellhead were closed. I went to Mr. Slaughters home to speak with him. He said that he made a handshake deal with Carrie Moore last year, after I had issued the compliance notice, where she agreed to have the well pumping by May 31, 2018. Mr. Slaughter said that no one has been out here since last year to do anything with the well. He also mentioned that he had requested to have the tanks moved, and nothing has been done with that. I tried to call Carrie Moore and she would not answer her phone. This well was required to be plugged and abandoned by 3/3/18. Nothing has been done to put this well back into production, and there has been no application submitted for a permit to plug this well. |

I made another attempt to call Carrie Moore. She did not answer so I left a voice message. I have made several attempts to contact Ms. Moore and I have not had any communication from her since June 2018. She will not answer her phone and she does not return my calls. |

I printed a copy of the notice and attempted to hand deliver it to Carrie Moore at her home/shop. I called he in the morning and got no answer. I drove to her address and found no one was in the garage/shop for KMFAD. I then drove up to the home. I found that Carries personal vehicle was parked in the driveway. I knocked on the front door and the lower door where the KMFAD sign was. There was no answer at either door. I called again and she did answer the phone, but she would not come to the door. I told her why I was there and I was leaving the compliance notice in the door where her business sign was. She acknowledged that she was aware of why I was there and she was aware of where I was leaving the notice. Carrie claimed that she would call me later in the day. |
