The tank battery consisted of one 100 barrel storage tank and one separator. Proper identification was posted and there were no leaks or spills. No violations at this time.

1387895402 2017-06-01 Production Wells No Violations This inspection was conducted in response to mailed correspondence from Critchfield, Critchfield and Johnston, attorneys representing Jeanne Bishop, which indicated that an Affidavit of Forfeiture of Oil and Gas Lease and a Notice of Failure to File and Request for Cancellation of Oil and Gas Lease had been filed in the official records of Coshocton County, Ohio. The letter stated that there is no longer production of oil or gas on the Bishop property, and specified that KMFAD is to plug and abandon this well within sixty days.

On the day of my inspection I found that the well was equipped with a natural gas powered pumping unit. The engine and drive belts were intact, and the engine fuel line was connected from the well casing. The flow lines were all connected at the wellhead. The valves were open from the tubing and casing. There was a gauge between the casing valve and the engine gas regulator that showed 250 psi and the gauge on the tubing line showed 50 psi. The polished rod was wet and shiny, and there was grease seeping from the top of the stuffing box. The production equipment consisted of one 100 barrel steel tank and one horizontal separator. There was also a house gas meter at the separator. There was not a containment dike around the equipment. The valves at the separator were open, and the gauge on the separator showed 50 psi. KMFAD Oil & Gas reported oil and gas production in 2014 and 2015, but there were no production records shown for 2016. I called Carrie Moore to ask about the well status. She claimed that the well was being produced and she referred me to the well tender Jim Dunfee for specific details. I called Jim and he said that he has been pumping this well since he was hired in September, 2016. Jim claimed that the well makes an average of 30 to 44 thousand cubic feet (MCF) of natural gas every month, and he currently has 3 feet 2 inches of oil in the tank. He also claimed that the well was making house gas for a cabin on the property. Th

1910216539 2019-01-18 Exempt Well Pre Transfer No Violations On the day of my inspection I found that the pumping unit had been removed. The well was equipped with tubing and rods. The flow lines were connected at the wellhead, but both the tubing and casing valves were closed. The 100 barrel tank and separator had both been removed. The domestic gas meter and drip bottle were still in place, but the flow line was disconnected, the valves were closed and there was zero (0) pounds of gas pressure on the gauge. There was no identification posted at the wellhead. The ground was snow covered and I did not find any signs of a fluid leak in the snow. I did not find any gas leaks around the wellhead. I tried to call Carrie Moore to ask about the well; she did not answer her phone and her voice mailbox was full. I also tried to call the landowner, Mrs. Bishop two times and she did not answer. I left messages both times, but she did not return my calls. I did get ahold of the former pumper Jim Dunfee and he said that there was an existing domestic gas line that was plumbed from the meter back to the farm house to the west. I drove back to the house, but it was not occupied and there were no vehicle tracks in the snow. Jim said that it was a weekend home. He also said that they would need to plumb the flow line to the domestic gas meter to get gas back to the house. This well meets the following criteria under ORC 1509.01(X)(1): (1) Anita Jeanne Bishop is the owner of the surface estate of the tract on which the well is located; (2) The well is located more than two hundred feet horizontal distance from any inhabited private dwelling house other than an inhabited private dwelling house located on the tract on which the well is located; (3) The well is located more than two hundred feet horizontal distance from any public building that may be used as a place of resort, assembly, education, entertainment, lodging, trade, manufacture, repair, storage, traffic, or occupancy by the public. This well does not meet the criteria of being used
