Mark claimed that he replaced the packing around the 2.875 casing; he replaced the bolts on the wellhead and tighten the top down. I did not detect any gas leaking from the wellhead and I did not observe any bubbling in the water that had collected around the surface casing. The oily pads had been removed from inside of the dike. The bare area in front of the dike had been seeded and covered with straw. The area was growing with new grass. This violation is resolved. |

I was joined by SE Regional Supervisor Brent Bear and NE Regional Emergency Operations Manager Terry Whitehair. On the day of our inspection we found that the well was being produced with a casing plunger system. The surface casing head was below grade and not visible. There was continuous bubbling from the damp soil around the wellhead. The tank battery consisted of one 50 barrel steel tank and one horizontal separator. Identification was posted at the tank battery. There was visual evidence of a previous spill at the tank battery, such as oil stained soil and oil soaked absorbent pads inside of the dike. The dike walls appeared to be recently constructed, fairly stable and questionable as to the capability of containing fluids. There was also a 15 x 20 bare area outside of the dike. Mark Fluharty, pumper for Northwood, claimed that the bare area was a result of excavation to build up the dike walls. The issues needing addressed at this well to bring it into compliance include: identifying and correcting the source of the gas leaking around the wellhead, removing the oil soaked pads, remediating the bare soil in front of the dike in order to establish vegetative cover. It is also recommended that the walls of the earthen dike be reinforced to provide secondary containment in the case of a fluid release. |
