1548171628 2019-01-22 Exempt Well Pre Transfer No Violations I conducted an exempt domestic well pre-transfer inspection on 1/17/19 as requested. Upon review of the well location and surrounding structures I found that the well in question for transfer was potentially located less than two hundred feet horizontal distance from a public building that may be used as a place of assembly, trade, or occupancy by the public. I contacted DOGRM surveyor Tom Smith to assist me with accurately determining the distance from the well to the corner of a local grocery/supply store. On the day of my inspection I met Tom at the well site. He collected data points on the grocery store and the wellhead. Tom found that the well was only 100 feet from the building. This well does not meet the criteria for an exempt domestic well, as defined under ORC 1509.01(X)(1), because it does not meet the 200 foot spacing requirement.
1567079879 2019-01-17 Exempt Well Pre Transfer No Violations On the day of my inspection I found that the well was equipped with a plunger lift and it was plumbed for domestic gas. The lines were connected, the valve was open, and the gauge showed gas pressure. There were no audible gas leaks at the wellhead. The ground was snow covered and I did not find any signs of fluid leaking from the wellhead in the snow. The production equipment consisted of one 210 barrel steel tank, one vertical separator and an underground concrete brine vault. I did not find any signs of leaking or contamination around the production equipment site. There was no identification posted at the tank or wellhead. I met with Mr. Hershberger and he showed me that the well is plumbed to his home and a shop on the opposite side of the township road. I told Mr. Hershberger that he needs to get new identification posted within 60 days of assuming ownership, and I gave him a list of the identification requirements. This well meets the following criteria under ORC 1509.01(X)(1) Roy Hershberger is the owner of the surface estate of the tract on which the well is located; (2) The well is used primarily to provide gas for the owners domestic use; (3) The well is located more than two hundred feet horizontal distance from any inhabited private dwelling house other than an inhabited private dwelling house located on the tract on which the well is located. The one discrepancy that I found upon reviewing the DOGRM well locator map was that the well is located less than 200 feet horizontal distance from a public building that may be used as a place of assembly, trade, or occupancy by the public. The well locator map measurement tool showed the well as being 141 feet from the corner of a grocery/supply store. I notified Bill Grubaugh and Kathy Smith. I also contacted DOGRM surveyor Tom Smith to schedule a meeting to shoot an accurate distance from the wellhead to the grocery store.
